心理护理干预对老年慢性心力衰竭患者情绪及心功能影响的临床分析

Clinical analysis of psychological nursing intervention on mood and cardiac function in elderly patients with chronic heart failure

目的探讨心理护理干预对老年慢性心力衰竭患者情绪及心功能的影响。方法 96例老年慢性心力衰竭患者作为研究对象,随机分为对照组与观察组,各48例。对照组给予常规护理,观察组在常规护理的基础上加用心理护理干预。对比两组患者干预前后的焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、脑钠肽(BNP)、左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)。结果干预前,两组患者的SAS评分、SDS评分对比,差异均无统计学意义(P>0.05);干预后,两组患者的SAS评分、SDS评分均较本组干预前降低,且观察组SAS评分(44.22±1.57)分、SDS评分(43.24±1.43)分低于对照组(46.83±1.85)、(47.96±1.86)分,差异均具有统计学意义(P<0.05)。干预前,两组患者的BNP、LVEF、LVEDD、LVESD对比,差异均无统计学意义(P>0.05);干预后,两组患者的BNP、LVEF、LVEDD、LVESD均较本组干预前改善,且观察组改善幅度大于对照组,差异均具有统计学意义(P<0.05)。结论将心理护理干预应用到老年慢性心力衰竭患者治疗护理中效果显著,可有效缓解患者的负性情绪,协同提高患者的心功能,改善患者的预后,值得临床推广使用。

Objective To discuss the effect of psychological nursing intervention on mood andcardiac function in elderly patients with chronic heart failure.Methods A total of96elderly patients withchronic heart failure as study subjects were randomly divided into control group and observation group,with48cases in each group.The control group received conventional nursing,and the observation group receivedpsychological nursing intervention on the basis of the control group.Comparison were made on self-ratinganxiety score(SAS),self-rating depression scale(SDS)score,brain natriuretic peptide(BNP),left ventricularejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD),left ventricular end-diastolicdiameter(LVESD)before and after intervention in two groups.Results Before intervention,both groupshad no statistically significant difference in SAS and SDS score(P>0.05).After intervention,both groups hadlower SAS and SDS score than before intervention,and the observation group had lower SAS score as(44.22±1.57)points and SDS score as(43.24±1.43)points than the control group[(46.83±1.85)and(47.96±1.86)points],and their difference was statistically significant(P<0.05).Before intervention,both groupshad no statistically significant difference in BNP,LVEF,LVEDD and LVESD(P>0.05).After intervention,both groups had better BNP,LVEF,LVEDD and LVESD than before intervention,and the observation grouphad greater improvement range than the control group.Their difference was statistically significant(P<0.05).Conclusion Application of psychological nursing intervention provides remarkable effect in intervention andnursing of elderly patients with chronic heart failure,and it can effectively alleviate the negative emotion ofpatients,improving the patient’s heart function and improve the prognosis of patients.It is worthy of clinicalpromotion and use.
