格列美脲治疗老年2型糖尿病的临床应用意义探究

Investigation on clinical application significance of glimepiride in the treatment of elderly type 2 diabetes mellitus

目的研究格列美脲治疗老年2型糖尿病的临床应用意义。方法 200例老年2型糖尿病患者,按照随机数字表法分为对照组及格列美脲组,各100例。对照组单一采用甘精胰岛素进行治疗,格列美脲组在对照组基础上采用格列美脲进行治疗。比较两组患者治疗前后空腹血糖水平、餐后2 h血糖、糖化血红蛋白水平,治疗效果及低血糖反应、体重增加发生率。结果治疗前,两组患者的空腹血糖水平、餐后2 h血糖及糖化血红蛋白水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的空腹血糖水平、餐后2 h血糖及糖化血红蛋白水平均较本组治疗前降低,且格列美脲组降低程度优于对照组,差异均具有统计学意义(P<0.05)。格列美脲组治疗总有效率为95.00%,高于对照组的79.00%,差异具有统计学意义(P<0.05)。格列美脲组低血糖反应、体重增加发生率为4.00%,低于对照组的20.00%,差异具有统计学意义(P<0.05)。结论格列美脲治疗老年2型糖尿病的临床应用效果确切,可有效降低糖化血红蛋白、空腹血糖、餐后2 h血糖水平,维持血糖平稳,改善患者预后,且可减少低血糖的发生,提高用药安全性,适合老年2型糖尿病患者用药。

Objective To study the clinical application significance of glimepiride in the treatment of elderly type2diabetes mellitus.Methods A total of200elderly patients with type2diabetes mellitus were divided by random number method into control group and glimepiride group,with100cases in each group.The control group was treated with insulin glargine only,and the glimepiride group was treated with glimepiride on the basis of the control group.Comparison were made on fasting blood glucose levels,postprandial2h blood glucose,glycosylated hemoglobidifference was statistically significantn levels,treatment effects,incidence of hypoglycemia reaction and weight gain before and after treatment between two groups.Results Before treatment,both groups had no statistically significant difference in fasting blood glucose levels,postprandial2h blood glucose and glycosylated hemoglobin levels(P>0.05).After treatment,both groups had lower fasting blood glucose levels,postprandial2h blood glucose and glycosylated hemoglobin levels than before treatment,and the glimepiride group had better decline degree than the control group.Their difference was statistically significant(P<0.05).The glimepiride group had higher total treatment effective rate as95.00%than79.00%in the control group,and the difference was statistically significant(P<0.05).The glimepiride group had lower incidence of hypoglycemia reaction and weight gain as4.00%than20.00%in the control group,and their difference was statistically significant(P<0.05).Conclusion Glimepiride shows affirmative clinical application effect in treating elderly type2diabetes mellitus,and it can effectively reduce the glycosylated hemoglobin,fasting blood glucose,postprandial2h blood glucose level,keep blood sugar stable,improve the prognosis of patients,and reduce the occurrence of hypoglycemia,improve medication safety.It is suitable for medication of elderly patients with type2diabetes.
