闭合复位弹性钉内固定治疗成人桡尺骨骨干骨折 被引量:3

摘要 目的观察和分析闭合复位弹性钉内固定治疗成人桡尺骨骨干骨折的方法及效果。方法选择本院接收治疗的120例成人桡尺骨骨干双骨折患者为研究对象。随机分成两组,对照组实施切开复位有限接触动力加压接骨板内固定法进行治疗,实验组则采用闭合复位弹性钉内固定法进行治疗,比较、分析两组患者的术中出血量、手术时间、手术切口、住院时间;并发症(骨不连、切口感染、神经损伤);术后前臂旋转功能。结果在术中出血量、手术时间、手术切口、住院时间,对照组显著低于实验组,差异具有统计学意义(P<0.05)。并发症(骨不连、切口感染、神经损伤);对照组在术后出现并发症的有4例(6.66%),其中骨不连2例,切口感染2例,实验组术后桡神经浅支出现了受刺激症状2例(3.33%),在并发症发生率的比较中,实验组显著优于对照组,差异具有统计学意义(P<0.05)。术后前臂旋转功能评分情况比较,实验组的优良率为90.0%,对照组的优良率只有60.0%,,实验组明显优于对照组,差异具有统计学意义(P<0.05)。结论闭合复位弹性钉内固定治疗成人桡尺骨骨干骨折具有重要的临床应用价值,可以降低出血量,缩短手术时间,切口小、住院时间短;术后并发症发生率低;术后前臂旋转功能恢复良好。 Objective Method of screw fixation for the treatment of adult radial and ulnar shaft fracture and effect observation and analysis ofelastic closed reduction.Methods Our hospital receiving treatment in120cases of adult radial and ulnar shaft fracture patients as the research object.Were randomly divided into two groups,the control of open reduction and dynamic compression plate fixation for treatment of the patients,theexperimental group was treated with closed reduction and elastic nail internal fixation method,comparison and analysis of two groups of patientswith postoperative complications,recovery of function of patients.Results Intraoperative blood volume,surgical time,surgical incision,and hospitalizationtime were significantly lower in the control group than in the experimental group,and the difference was statistically significant(P<0.05).Complications(bone disconnection,incisional infection,nerve injury);Control group in the postoperative complications in4cases(6.66%),includingbone nonunion in2cases,infection of incision in2cases,the experimental group after the superficial branch of radial nerve is stimulated symptomsin2cases(3.33%),the incidence of complications,in the comparison of the experimental group was significantly better than the control group,difference has statistical significance(P<0.05).The optimal rate of the experimental group was90.0%compared with the rotation function score ofthe forearm,and the excellent rate of the control group was only about60.0%,the experimental group was significantly better than the control group,with statistical significance(P<0.05).Conclusion The effect of closed reduction with elastic nail fixation for the treatment of adult patients withradial and ulnar shaft fracture,which has important value in clinical application,can effectively alleviate the patient's condition,effective treatment ofadult ulnar and radial fracture can shorten operation time,reduce the amount of bleeding,reduce the incidence of complications and improve the lifequality of the patients.Worthy of reference.
