生物反馈治疗高龄老年人功能性便秘的疗效观察

Clinical Effect Observation On Biofeedback Therapy in Treatment of Very Elderly Patients With Functional Constipation

目的本研究旨在观察生物反馈治疗高龄老年人功能性便秘的临床疗效。方法共入选高龄老年人慢性功能性便秘患者98例,根据治疗方法的不同,随机分为对照组(n=50)和实验组(n=48),对照组给予乳果糖15 ml,2次/天进行治疗,实验组给予生物反馈治疗,4周后比较两组治疗总有效率,应用肛门直肠测压方法比较实验组治疗前后肛管静息压、直肠静息压、排便阈值、感知阈值、最大耐受量的变化。结果实验组治疗总有效率高于对照组(68.75%vs.36.00%,P<0.05);实验组治疗后肛管静息压、直肠静息压、直肠排便阈值、直肠感知阈值、最大耐受量均较治疗前有明显改善(P<0.05)。结论生物反馈治疗高龄老年人功能性便秘疗效确切,其机制可能与其改善肛门直肠动力学有关。

Objective To observe therapeutic effect of biofeedback therapy on very elderly patients with functional constipation(FC).Methods98cases of very elderly patients with FC were enrolled in the study.All the patients were randomly divided into control group(n=50)and experimental group(n=48)randomly according to different treatment methods.Control group was treated by Lactulose15ml,twice a day,while experimental group was treated by biofeedback therapy.The total effective rate were compared between the two groups.The anal canal resting pressure,rectum resting pressure,rectal defecation threshold,rectal perception threshold,and maximum tolerated volume were evaluated by vectorial manometry between posttreatment and pretreatment in experimental group.Results The total effective rate in experimental group was significantly higher than that in control group(68.75%vs.36.00%)(P<0.05).After treatment,the levelof anal canal resting pressure,rectum resting pressure,rectal defecation threshold,rectal perception threshold,and maximum tolerated volume in experimental group were decreased significantly compared with that of pre-therapy(P<0.05).Conclusion The treatment of biofeedback therapy for very elderly patients with functional constipation was effective and its mechanism may be related to the improvement of anorectum dynamics.
