3.0T磁共振不同序列诊断膝关节滑膜炎的对照研究 被引量:11

A control study of different sequences of 3.0T MRI system in diagnosis for gonarthromeningitis

摘要 目的:通过对比3.0T磁共振成像(MRI)三维可变翻转角快速自旋回波(3D-SPACE)双反转恢复(DRI)3D-Space-DRI序列、T_1WI、T_2WI和质子密度加权成像(PDWI)序列在膝关节滑膜炎中滑膜的对比噪声比(CNR),从而确定显示滑膜最佳序列。方法:对60例临床怀疑膝关节滑膜炎的患者行膝关节T_1WI、T_2WI、PDWI和3D Space-DRI序列扫描,测量各序列图像滑膜与滑膜腔积液和滑膜周边脂肪组织的CNR,分别为CNR1和CNR2,并对3D Space-DRI、T_2WI图像的均匀度、滑膜显示程度及图像整体质量进行双盲主观评分。图像的客观指标采用单变量方差分析(ANOVA)进行统计分析,主观指标采用Wilcoxon秩和检验进行统计分析。结果:3D SpaceDIR、T_1WI、T_2WI和PDWI序列的CNR1分别为55.33±7.88、5.13±2.96、34.91±2.27和40.61±6.58;CNR2分别为44.38±1.22、30.10±0.24、26.86±0.67和9.23±0.44。3D SpaceDIR、T_2WI均匀度主观评分结果为(4.36±0.08)分、(4.44±0.07)分;对滑膜显示程度主观评分结果为(4.70±0.06)分、(2.05±0.03)分;图像整体质量评分为(1.23±0.06)分、(4.51±0.07)分。结论:3.0T磁共振3D Space-DIR序列对膝关节滑膜炎时滑膜形态的显示更有优势。 Objective:To confirm the optimal sequence of displaying synovial membrane through compare thecontrast noise ratio(CNR)of different sequences MRI for synovial membrane of gonarthromeningitis.Methods:60patients with suspected gonarthromeningitis were scanned on knee-joint by using a series of sequences includingT1WI,T2WI,PDWI and3D space-DRI,respectively.And the CNRs between synovium and effusion of synovialcavity and between synovium and adipose tissue around synovium of different sequences were detected,and theywere recorded as CNR1and CNR2,respectively.Besides,double-blind subjective scores were applied to3D Space-DRI,uniformity of T2WI,display degree of synovium and the total quality of image.And ANOVA was adopted toanalyze the objective indexes of image,and Wilcoxon rank sum test was used to analyze objective index.Results:The CNR1s of3D Space-DIR,T1WI,T2WI,PDWI were55.33±7.88,5.13±2.96,34.91±2.27and40.61±6.58,respectively.And the CNR2s of them were44.38±1.22,30.10±0.24,26.86±0.67and9.23±0.44,respectively.Thesubjective scores about image uniformity of3D Space-DIR and T2WI were4.36±0.08and4.44±0.07,respectively.And the subjective scores about display degree of synovial membrane were4.70±0.06and2.05±0.03,respectivley.Besides,the total quality scores of image were1.23±0.06and4.51±0.07,respectively.Conclusion:3D Space-DIRsequence of3.0T MRI has more advantages in the diagnosis of synovial morphology for gonarthromeningitis.
