彩超联合ABI对中年高血压缺血性脑卒中的预测及颅脑MR的对照

Application of Carotid Artery Ultrasound Combined with Ankle Brachial Index and Contrast Cerebral MR Inspection in Prediction of Middle Age Hypertensive Ischemic Stroke

目的应用颅脑MR对照分析颈动脉彩超联合踝肱指数(Ankle Brachial Index,ABI)对中年高血压缺血性脑卒中的预测价值。方法选取2016年1月~12月间入住我院神经内科的高血压缺血性脑卒中患者50例,非脑卒中患者50例,分别行颈动脉彩超检查、ABI检测及两者联合检测,并与颅脑MR结果进行对照,对检测结果进行统计学处理。结果脑卒中组患者的总胆固醇(Total Cholesterol,TC)和低密度脂蛋白胆固醇(Low Density Lipoprotein Cholesterol,LDL-C)均高于非脑卒中组(差异有统计学意义P<0.05)、吸烟患者数量明显多于非脑卒中组(差异具有高度统计学意义P<0.01)。脑卒中组患者的颈动脉IMT增厚、颈动脉斑块的发生率及ABI的异常均明显高于非脑卒中组。颈动脉彩超联合ABI检测的ROC曲线下面积显著大于单纯颈动脉彩超检查或单纯ABI检测。结论降脂、戒烟、定期进行颈动脉彩超和ABI联合检查可更为有效地防止和预测中年高血压患者缺血性脑卒中的发生。

Objective To evaluate the application of carotid artery ultrasound combined with ankle brachial index(ABI)in predictionof middle age hypertensive ischemic stroke by contrasting cerebral MR inspection.Methods A total of50patients with hypertensiveischemic stroke from neurology in our hospital between January1,2016and December31,2016,and50hypertensive patients withoutischemic stroke were chosen in this study.All the patients in the treatment group underwent cerebral MR inspection,carotid arteryultrasound,ABI detection,and carotid artery ultrasound combined with ankle brachial index detection.While those in the control groupwere treated with cerebral MRI.The detection results were analyzed statistically.Results The total cholesterol(TC)and low densitylipoprotein cholesterol(LDL)in stroke group were much higher than those in control group(P<0.05)and smokers were more thanthose in control group(P<0.01).The incidence of carotid artery IMT and carotid plaque in stroke group was significantly higher thanthat in control group.The area under ROC curve of carotid artery ultrasound combined with ABI was significantly larger than that ofsimple carotid artery ultrasound examination or simple ABI detection.Conclusion Reducing lipid,quit smoking,regularly carotid arteryultrasound combined with ABI examination can predict and prevent ischemic stroke more effectively for middle age hypertensive.
