“All-on-4”种植即刻修复治疗的临床回顾性分析

Immediate repair of All-on-4 implants: a clinical retrospective study

目的分析施行"All-on-4"种植即刻修复病例的种植体存留率及并发症发生情况,探讨"All-on-4"的技术要点及注意事项。方法根据纳入及排除标准,纳入2007年1月~2015年12月完成的"All-on-4"种植即刻修复治疗的病例共23例108枚种植体,其中上颌牙列缺失5例,下颌牙列缺失14例,全口牙列缺失4例,均在术后当天或1~2 d内取模,并于术后7~10 d戴入临时固定修复体,采用螺丝固位,术后3~6个月取模并制作最终修复体,共完成27件临时修复体及27件最终修复体。完成最终修复后平均追踪3年(1~9年),对种植体留存率、修复并发症进行总结及分析。结果 108枚种植体及27件最终修复体至最后1次复查为止,种植体存留率100%。23位患者当中,3位患者发生并发症,并发症发生率13.0%。结论 "All-on-4"在牙列缺失病例治疗中疗效确切可靠,并发症的发生率较低,是一种切实可行的方法。

Objective To analyze implant survival rate and incidence rate of complications after All-on-4implantation,and to discuss the technical points and consideration of All-on-4.Methods According to the criteria of inclusion and exclusion,23cases treated with All-on-4immediate restoration were referred,from January2007to December2015,including5cases of edentulous maxillary,14cases of edentulous mandibular and4cases of both maxillary and mandibular were edentulous.All the patients received immediate function with provisional prostheses at the surgery day.Definitive prostheses were delivered to patients after3-6months post operation and follow-up visits were performed up to9years(average3years)after placement of definitive prostheses.Results In this study,the rate of implant retention of108implants and27final restorations was100%until the last review.Among the23patients,3patients had complications and the incidence of complications was13.0%.Conclusion All-on-4is an effective and reliable method in the treatment of dentures denture.The incidence of complications is low.All?on?4is a practical method.
