高膳食纤维饮食干预对妇科肿瘤合并糖尿病患者围手术期血糖水平的影响

Effect of High Dietary Fiber Dietary Intervention on Perioperative Blood Glucose in Patients with Gynecological Cancer Complicated with Diabetes Mellitus

目的探讨高膳食纤维饮食干预对妇科肿瘤合并糖尿病患者围手术期血糖水平的影响。方法选取2014年10月~2016年10月我科收治的90例妇科肿瘤合并糖尿病患者,根据随机数字表法将患者分为观察组和对照组,各45例。对照组给予饮食宣教及糖尿病常规护理。观察组给予高膳食纤维饮食干预,干预时间为1周。观察两组围手术期血糖控制情况、并发症发生情况及术后康复情况。结果观察组干预后空腹血糖(FBG)、餐后2h血糖(2hPBG)水平均低于对照组(P<0.05)。观察组血糖控制良好率高于对照组(P<0.05),观察组胰岛素使用率、低血糖、围手术期感染、下肢静脉血栓发生率低于对照组(P<0.05)。观察组术后平均住院时间、下床活动时间短于对照组,患者满意度评分高于对照组(P<0.05)。结论高膳食纤维饮食干预可有效控制妇科肿瘤合并糖尿病患者围手术期血糖水平,并能有效降低患者胰岛素使用率及并发症发生率,可促进患者术后康复,缩短患者住院时间,提高患者满意度。

Objective To investigate the effects of high dietary fiber dietary intervention on blood glucose during perioperative period in patients with gynecological tumor and diabetes mellitus.Methods90cases of patients with gynecological tumors and diabetes were divided into observation group(n=45)and control group(n=45)according to the random number table from October2014to October2016.The control group were received with routine diet and diabetes routine.The observation group were received with nutrition programs on the basis of the nutritionist before the development.The observation group were accepted with dietary intervention was conducted under the guidance of dietitian.The intervention time was2weeks.The blood glucose control,complication and postoperative rehabilitation of two groups were observed.ResultsThe levels of fasting plasma glucose(FBG),2h postprandial blood glucose(2h PBG)of observation group were lower than control group(P<0.05).The rate of insulin use,hypoglycemia,perioperative infection and lower extremity venous thrombosis of observation group were lower than control group(P<0.05).The patients'satisfaction score of observation group was higher than control group(P<0.05).ConclusionDietary intervention can effectively control the blood glucose level of patients with gynecologic cancer and diabetes mellitus,and can effectively reduce the patient's insulin use rate and the incidence of complications.It can promote the rehabilitation of patients,shorten the hospital stay and improve patient satisfaction.
