开放清创联合Top-closure治疗糖尿病足坏疽疗效观察

Observation of combined with Top-closure in the treatment of diabetic foot gangrene effect of debridement

目的探讨开放清创联合皮肤牵张闭合系统(skin stretching securewound-closure system,Top-closure)治疗糖尿病足坏疽的效果。方法青岛大学附属海慈医院血管外科收治糖尿病足患者60例作为研究对象,在控制血糖、抗感染、改善血流、对症护理等综合治疗的基础上,应用开放清创联合Top-closure治疗,通过持续或间歇地多向牵张皮肤达到闭合创面的目的。结果 60例糖尿病足患者经治疗1~4个月后,创面肉芽组织生长新鲜,较常规换药平均愈合时间明显缩短。结论开放清创联合Top-closure治疗糖尿病足坏疽可明显缩短愈合时间,减少治疗费用,有效避免了患者高位截肢。

Objective To investigate the debridement and skin stretch closed system(skin stretching securewound-closure system,Top-closure)for the treatment of diabetic foot gangrene effect.Methods Sixty cases of diabetic foot patients as the research object,take control of blood glucose,anti infection,improving blood flow,based on the comprehensive treatment of symptomatic nursing,combined with Top-closure in the treatment of open application clear,through continuously or intermittently multidirectional stretch the skin to achieve the purpose of healing.Results Sixty cases of diabetic foot patients treated for1~4months after treatment,granulation tissue growth of fresh,more conventional dressing the average healing time was shortened obviously.Conclusion Combined with Top-closure in the treatment of diabetic foot gangrene can signifi cantly shorten the healing time of opening clear,reduce the cost of treatment,effectively avoid patients high amputation.
