CCL-18与CyclinD1分子在口腔颌面部恶性肿瘤患者血清中的变化及临床意义 被引量:2

Clinical Significance Study of CCL-18 and CyclinD1 Levels in Serum of Patients with Oral and Maxillofacial Malignant Tumor

摘要 目的分析口腔颌面部恶性肿瘤患者血清CCL-18和CyclinD1水平,探讨其与口腔颌面部恶性肿瘤的病理类型、淋巴转移及临床分期的相关性,并判断其临床诊断价值。方法选取在本院接受住院治疗的口腔颌面部恶性肿瘤患者128例为试验组,同期体检正常人群128例为对照组。手术前和手术后2周采集试验组患者空腹静脉血,对照组常规检查后采集空腹静脉血,ELISA法检测血清中CCL-18和CyclinD1的含量。结果试验组血清CCL-18和CyclinD1水平均较对照组高,手术后血清CCL-18和CyclinD1水平均较手术前明显降低;肿瘤临床分期为Ⅱ期的患者血清CCL-18水平较高,而Ⅳ期患者血清CyclinD1水平较高(P<0.05)。血清CCL-18和CyclinD1水平与口腔颌面部恶性肿瘤的类别无关(P>0.05)。CCL-18、CyclinD1及联合诊断口腔颌面部肿瘤的AUC值分别为0.759、0.760和0.822,联合检测诊断口腔颌面部肿瘤的效能明显优于各项指标单独诊断的价值。结论口腔颌面部恶性肿瘤患者血清CCL-18和CyclinD1水平较高,可能与肿瘤的发生发展有关,二者联合检测可能作为口腔颌面部恶性肿瘤的临床诊断指标。 Objective To analyze the significance of CCL-18and CyclinD1in serum of the patients with oral and maxillofacial malignanttumorr,and to explore the correlation among two molecules’level,the pathological reason,lymph node metastasis,clinical staging oforal and maxillofacial malignant tumor,and to analyze the value of their clinical diagnosis.Methods128patients with oral and maxillofacialmalignant tumor in our hospital and128normal people around the hospital were selected,and they were divided as experiment group andcontrol group respectively.Fasting venous blood before operation and two weeks after operation in the experimental group and fasting bloodafter routine examination in the control group were collected to test the levels of CCL-18and CyclinD1by enzyme-linked immunosorbentassay.Results The levels of CCL-18and CyclinD1in experiment group before operation were higher than that of control group.After surgery,the levels of CCL-18and CyclinD1decreased significantly.There was relationship among their levels,tumor clinical stage and lymphnode metastasis.CCL-18showed a higher level at phase II while CyclinD1at clinical stage IV,and the difference was statistically significant(P<0.05).The levels of CyclinD1and CCL-18had no relationship with the types of oral and maxillofacial malignant tumor,and thedifference was not statistically significant(P>0.05).The AUC values of CCL-18,CyclinD1and the combination detection of the two were0.759,0.760and0.822respectively for the diagnosis of oral and maxillofacial tumors.The efficacy of combined detection was superior tothat of the individual diagnosis for oral and maxillofacial tumors.Conclusion There were higher level of CCL-18and CyclinD1in malignanttumor which may relate to the occurrence and development of oral and maxillofacial malignant tumor.Combined detection of the two couldact as indicators of clinical diagnosis for oral and maxillofacial malignant tumor.
