细节护理管理对基层社区医疗机构感染的影响及实践效果评价

Influence of detailed nursing management on nosocomial infection of community medical institutions and its application effects

目的探讨细节护理管理在基层社区医疗机构的实施方法及应用效果,为基层护理实施及管理提供可借鉴依据。方法收集2015年1月~6月期间实施常规护理的患者712例为常规护理组,收集2016年1月~6月实施细节护理管理的患者705例为细节护理组,比较两组患者院内感染发生率及护理满意度评分差异,并对比分析实施前后护理人员的无菌观念及操作细节的8项细节评分差异。结果细节护理组院内感染的总发生率显著低于常规护理组(P<0.05);细节护理组中护理人员在手卫生、消毒剂配置及无菌物品处理等6个细节执行情况评分均显著高于常规护理组(P<0.05);细节护理组患者满意度评分(就诊接待、环境介绍及安全宣教等14个项目)均显著高于常规护理组(P<0.05)。结论细节护理管理有利于降低院内感染发生率,规范护理人员操作,提高患者护理满意度,推荐社区医疗机构应用。

Objective To discuss the implementation methods and its application effects of detailed nursing management in community medical institutions,and for providing evidences for community medical nursing.Methods Totally712patients underwent routine nursing were collected and included into routine nursing group from January to June2015,and705patients underwent detailed nursing management were collected and included into detailed nursing group from January to June2016.The incidence of nosocomial infections and satisfaction to nursing between two groups were compared,and the scores of the aseptic concepts and operation details of nurses were analyzed and compared before and after undergoing detailed nursing management.Results The total incidence of the detailed nursing group was significantly lower than that of the routine nursing group(P<0.05).The score of nurses in six details(such as including hands sanitary managements,sanitizers allocation and aseptic objects.)of the detailed nursing group were significantly higher than those in the routine nursing group(P<0.05).The satisfaction scores of detailed nursing group(including14items such as receptions,environmental introduction and safety education,etc.)were significantly higher than those in the routine nursing group(P<0.05).Conclusion The detailed nursing management can help reduce the incidence of infections,standardize the operation of nurses,increase the patients′satisfaction for nursing,and which is worthy of clinical application in community medical institutions.
