现代伤口处理模式下换药术联合介入手术治疗糖尿病足相关性溃疡的临床应用

Clinical application of modern wound therapy combined with interventional therapy for diabetic foot related ulcer

目的观察现代伤口处理模式下换药术联合介入手术治疗糖尿病足相关性溃疡的临床疗效。方法将63例糖尿病足溃疡患者随机分为对照组(31例)和研究组(32例),对照组患者给以传统换药术联合介入治疗,研究组患者给予现代伤口模式下换药术联合介入治疗,比较两组患者临床疗效及白细胞计数(WBC)、血沉(ESR)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、住院费用、溃疡面积、创面愈合时间及截肢率。结果研究组患者治疗总有效率(84.38%)明显高于对照组总有效率(58.06%),差异有统计学意义(Х2=5.342,P<0.05)。与治疗前比较,两组患者治疗后WBC、ESR、TC、TG、LDL-C、溃疡面积均值明显降低,差异具有统计学意义(P<0.05)。与对照组治疗后比较,研究组治疗后WBC、ESR、溃疡面积均值显著降低,差异具有统计学意义(P<0.05)。与对照组比较,研究组创面愈合时间、住院费用、截肢率显著降低,差异具有统计学意义(P<0.05)。结论现代伤口处理模式联合介入治疗在糖尿病足溃疡患者临床疗效显著,不仅改善了伤口愈合的基本条件,且促进了溃疡愈合的整体效果,减轻了患者的医疗负担,值得临床借鉴应用。

Objective To observe the clinical effect of changing dressings combined with interventional surgery in the treatment of diabetic footrelated ulcers.Methods The control group included63cases of patients with diabetic foot ulcer were randomly divided into study group(31cases)and control group(32cases),patients were treated with conventional dressing combined with interventional therapy,patients in study group were givenunder the mode of operation of modern wound dressing combined with interventional therapy,and the clinical efficacy of the two groups of whiteblood cell count(WBC),erythrocyte sedimentation rate(ESR),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),cost of hospitalization,area of ulcer,wound healing time and the amputation rate.Results The total effective rate of the study group(84.38%)was significantly higher than that of the control group(58.06%),the difference was statistically significant(c2=5.342,P<0.05).Compared with beforetreatment,the mean value of WBC,ESR,TC,TG,LDL-C and ulcer area in the two groups were significantly lower,the difference was statisticallysignificant(P<0.05).Compared with the control group after treatment,the study group after treatment WBC,ESR and ulcer area mean decreasedsignificantly,the difference was statistically significant(P<0.05).Compared with the control group,the wound healing time,hospitalizationexpenses and amputation rate of the study group decreased significantly,the difference was statistically significant(P<0.05).Conclusion Modernwound treatment mode combined with interventional therapy in the treatment of diabetic foot ulcers in patients with significant clinical effect,not onlyto improve the basic conditions of wound healing,and promote the overall effect of ulcer healing,reduce the medical burden of patients,which isworthy of clinical application.
