乳腺钼靶和超声检查对于诊断乳腺癌的价值对比 被引量:16

摘要 目的为了评估高频超声、钼靶和联合检查对于诊断乳腺癌的意义.方法 2015年12月至2016年12月从昆明医科大学附属甘美医院门诊病人共12 057例病例中,筛选出在昆明医科大学附属甘美医院行术前影像学检查,术后病理证实为乳腺癌的患者共355例.对该355份病历进行回顾性分析,355名患者均行超声及钼靶诊断随机分为3组,单独统计超声结果的有125例患者,单独统计乳房钼靶结果的有103例患者,统计组合检测结果的有127例患者.结果钼靶X线检查的符合率为79.4%,误诊率为10.7%,和漏诊率为9.8%;高频超声检测的符合率80.0%,误诊率为11.2%,漏诊率为4.6%;2种检查结合的符合率是89.7%,误诊率为6.3%,漏诊率为4.0%.2种检查组合应用和单一应用高频超声和单一应用钼靶检查相比,灵敏度和漏诊率有统计学差异.误诊率无统计学差异.结论钼靶X线检查和高频超声分别有自己的优势.组合应用可以使2个检查优势互补,显著提高了检出率和准确率,降低乳腺癌误诊率和漏诊率. Objective To evaluate diagnostic significance of single high frequency ultrasound and mammography and combined therapy of both on breast cancer.Methods355cases of female breast cancer were selected from The First Hospital of Kunming from December2015to December2016.Among these355patients,125cases had only performed high frequency ultrasound,103cases had only performed breast X-ray radiography,127cases of conducted the therapy of both.Results The coincidence rate of singlebreast X-ray was79.4%,the misdiagnosis rate was10.7%,the missed diagnosis rate was9.8%.The coincidence rate of single high-frequency ultrasonic testis80.0%,the misdiagnosis rate was11.2%,the missed diagnosis rate was4.6%;the coincidence rate of combination of two test was89.7%,the misdiagnosis rate was6.3%,the missed diagnosis rate was4.0%.The detection rate and the missed diagnosis of joint diagnosis had statistical difference with single high frequency ultrasound and single mammography.There was no statistical difference on misdiagnosis rate.Conclusions Both high frequency ultrasound and mammography has their own advantages respectively.The joint application of both has better diagnosis complementary,and can significantly improve the detection rate and the accuracy rate of breast cancer,decrease the rate of misdiagnosis and missed diagnosis rate.
