山莨菪碱足三里穴位注射联合指压内关用于结肠镜检查镇痛临床观察

Clinical Observation of 654-2 Acupoint Injection at Zusanli Combined with Shiatsu at Neiguan Acupoint for Colonoscopy Analgesia

目的观察山莨菪碱足三里穴位注射联合指压内关用于结肠镜检查的镇痛效果。方法采用随机数字表法将140例行结肠镜检查的患者分为观察组和对照组各70例。观察组于结肠镜检查前15 min予山莨菪碱注射液,足三里穴位注射,每侧5 mg;指压内关,待患者感觉酸、胀、麻开始检查,至结肠镜到达回盲部停止按压。对照组于结肠镜检查前10 min予山莨菪碱注射液10 mg,肌肉注射。比较2组患者结肠镜检查时不同部位的疼痛程度评分、检查时间及满意度,观察结肠镜检查前后患者血清β-内啡肽(β-EP)和P物质(SP)水平。结果观察组在插镜时、过乙状结肠、过结肠曲时疼痛程度评分显著低于对照组(P<0.001),检查时间短于对照组(P<0.001),患者满意度评分高于对照组(P<0.001);检查后血清β-EP水平高于对照组(P<0.001),SP水平低于对照组(P<0.001)。结论结肠镜检查前应用山莨菪碱足三里穴位注射联合指压内关可调节血清β-EP、SP水平,镇痛效果明显,能缩短检查时间。

Objective To investigate the effects of654-2acupoint injection at Zusanli combined with shiatsu at Neiguan acupoint for colonoscopy analgesia.Methods Totally140patients underwent colonoscopy were divided into observation group and control group according to the random number table method,with70cases in each group.The observation group were given654-2acupoint injection at Zusanli(5mg each side)combined with shiatsu at Neiguan acupoint(the patient to feel the acid,swelling,hemp began to check,until colonoscopy to return to the ileocecum,stop pressing),and the control group were given654-2muscle injection(10mg)10min before colonoscopy.The pain scores,checking time and patients'satisfaction degree of the two groups were compared,and serum beta-endorphin(β-EP)and substance P(SP)level of the two groups before and after examination were compared.Results The pain degrees during colonlscope inserting and passing the sigmoid,splenic flexure of colon of the observation group were significantly lower than those of the control group(P<0.001);testing time was less than that of the control group(P<0.001);patient satisfaction scores were higher than those of the control group(P<0.001);serumβ-EP level after examination was higher than that of the control group(P<0.001);SP level was lower than that of the control group(P<0.001).Conclusion654-2acupoint injection at Zusanli combined with shiatsu at Neiguan acupoint for colonoscopy analgesia can adjust the serum levels ofβ-EP and SP,with obvious analgesic effect,which can shorten the testing time.
