血液灌流连接方式治疗尿毒症肾性脑病的临床观察及护理

Clinical observation and nursing of hemoperfusion in treatment of uremic renal connection encephalopathy

目的分析总结血液灌流连接方式治疗尿毒症肾性脑病的临床效果及护理。方法选取在本院接受治疗的尿毒症合并肾性脑病患者110例,并按照随机数字表法分为A组与B组,每组均55例。B组患者采用透析与灌流同时进行的常规治疗,A组采用透析2 h后连接灌流器的治疗方案,在整个治疗过程中,两组患者均给予精细护理。对比两组患者的不良反应发生率及治疗前后血肌酐、血尿素氮、血甲状旁腺素、血钾及血β2微球蛋白情况。结果 A组不良反应发生率明显低于B组,差异有统计学意义(P<0.05);A组血肌酐(332.89±30.66)μmol/L、血尿素氮(9.95±3.88)mmol/L、血甲状旁腺素(87.56±24.45)pg/m L、血钾(4.05±0.74)mmol/L及血β2微球蛋白(28.27±11.25)mg/L等均明显低于B组血肌酐(485.56±30.58)μmol/L、血尿素氮(15.68±3.07)mmol/L、血甲状旁腺素(251.05±104.33)pg/m L、血钾(4.46±1.32)mmol/L及血β2微球蛋白(50.81±12.32)mg/L,差异有统计学意义(P<0.05)。结论血液透析2 h后连接灌流器的治疗方案对毒素的清除率更高,配合精细护理后,明显缩短了操作时间,提高了病患的治疗依从性。

Objective To analyze the clinical effect of hemoperfusion connection treatment of uremia renal encephalopathy and nursing.Meth?ods110cases of patients with uremia complicated with renal encephalopathy in our hospital for treatment,and randomLy divided into group A and group B,55cases in each group.Dialysis and perfusion simultaneously with group B,group A using2h after hemodialysis connection perfusion dialysis therapy,in the whole course of treatment,two groups of patients are given careful nursing.The adverse reactions are compared between two groups before and after treatment and the incidence rate of serum creatinine,blood urea nitrogen,serum parathyroid hormone,serum potassium and serumβ2microglobulin are compared.Results Adverse reaction rate of group A was significantly lower than group B,with statistically significant(P<0.05);serum creatinine(332.89±30.66)μmol/L,blood urea nitrogen(9.95±3.88)mmol/L,serum parathyroid hormone(87.56±24.45)pg/mL,blood potassium(4.05±0.74)mmol/L and serumβ2microglobulin(28.27±11.25)mg/L of group A were significantly lower than blood creatinine(485.56±30.58)μmol/L,blood urea nitrogen(15.68.2±3.07)mmol/L,serum parathyroid hormone(251.05±104.33)pg/mL blood potassium(4.46±1.32)mmol/L and serumβ2microglobulin(50.81±12.32)mg/L of group B,with statistically significant(P<0.05).Conclusion The clearance rate of the toxin in the treatment of2h after hemodialysis is higher,and the operation time is shortened,and the treatment compliance of patients is improved after the treatment with fine nursing care.
