鼓膜双孔穿刺联合鼓室注药治疗分泌性中耳炎疗效分析

Curative effect analysis of double puncture of tympanic membrane combined with tympanic cavity injection in the treatment of secretory otitis media

目的分泌性中耳炎患者应用鼓膜双孔穿刺联合鼓室注药治疗的临床疗效与听力水平。方法选择本院耳鼻喉科2010年1月~2016年12月收治的48例分泌性中耳炎患者,按照随机数字表法将其分为穿刺组(24例,鼓膜双孔穿刺)、联合组(24例、鼓膜双孔穿刺+鼓室注药),并对比分析其临床疗效与听力效果。结果两组患者治疗前的气导听阈均值、骨导听阈均值、气-骨导听阈均值相比无统计学意义,治疗后,两组患者的听阈检测值均低于治疗前,且联合组患者的变化更为显著,且差异存在统计学意义(P<0.05)。联合组患者治疗后的临床总有效率为95.83%,高于穿刺组患者的79.17%,且差异存在统计学意义(P<0.05)。结论在鼓膜双孔穿刺基础上联合鼓室注药,可以显著提高分泌性中耳炎患者的临床治疗效果,缓解临床病症,改善患者各频率听阈水平,促进咽鼓管功能恢复,值得临床应用。

Objective To study the clinical curative effect and the hearing level of the patients with secretory otitis media by using the double puncture combined with tympanic cavity injection.Methods The Department of ENT in our hospital from January2010to December201648cases of patients with secretory otitis media,were randomLy divided into puncture group(24cases,tympanic membrane puncture holes)and combination group(24cases,tympanic membrane puncture and double tympanic administration),and comparative analysis of the clinical curative effect and the effect of hearing.Results Two groups of patients before treatment,the mean air conduction threshold,bone conduction threshold,the mean air bone conduction threshold average compared with no statistical significance.After the treatment,the hearing threshold of two groups of patients were lower than those before treatment,and the changes of patients in the combination group is more significant,and the difference in statistical significance(P<0.05).The total effective rate of the treatment group was95.83%,which was higher than that of the puncture group,and the difference was statistically significant(P<0.05).Conclusion The tympanic membrane puncture combined with double tympanic injection can significantly improve the clinical treatment effect of patients with secretory otitis media,alleviate clinical symptoms,improve the frequency threshold level of patients,promote the recovery of eustachian tube function,it is worthy of clinical application.
