AIRVO2型呼吸湿化治疗仪治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床效果研究

Clinical effectiveness of AIRVO2 respiratory humidification therapeutic apparatus in the treatment of chronic obstructive pulmonary disease complicated with type-Ⅱ respiratory failure

目的:探讨对慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者实施呼吸湿化治疗仪治疗的临床有效性与安全性。方法:回顾性分析70例慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者的临床资料,按照患者入院时间将其随机分为对照组和观察组,每组35例。对照组实施常规治疗;观察组在对照组基础上联合实施呼吸湿化治疗仪治疗。对比分析两组患者的治疗效果。结果:两组经不同方案治疗后,各项动脉血气水平检测指标均得到明显的改善,治疗后两组的动脉血氧分压(PaO_2)、动脉二氧化氮分压(PaCO_2)动脉血气水平和p H值的指标比较,其差异均有统计学意义(t=6.317,t=7.251,t=6.347;P<0.05);两组心率和呼吸频率测量水平与治疗前比较均显著下降,且治疗后两组间比较,其差异均有统计学意义(t=5.984,t=6.034;P<0.05);两组治疗有效率评估,观察组总有效率为94.29%,对照组为80.00%,两组比较差异有统计学意义(x^2=9.104,P<0.05)。结论:对在常规治疗的基础上联合实施呼吸湿化治疗仪治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者可以获得更为理想的效果,显著改善患者的临床指标,并具有良好的安全性。

Objective:To investigate the clinical effectiveness of AIRVO2respiratory humidification apparatus in the treatment of chronic obstructive pulmonary disease(COPD)complicated with type II respiratory failure.Methods:The clinical data of70patients with chronic obstructive pulmonary disease complicated with type II respiratory failure were collected and retrospectively analyzed.According to the time of admission,the patients were randomly divided into two groups,the control group(n=35)was treated with conventional therapy,and the observation group(n=35)was treated with AIRVO2respiratory humidification therapy on the basis of the method of control group.And the curative effectiveness of the two groups were compared and analyzed.Results:After treatment,each arterial blood gas index was obviously improved,and the differences of PaO2,arterial blood gas level of PaCO2and indicator of pH between the two groups were significantly(t=6.317,t=7.251,t=6.347,P<0.05),respectively.The heart rate and respiratory rate of the two groups of post-operation were significantly lower than that of pre-operation(t=5.984,t=6.034,P<0.05),respectively.Besides,the total effective rate of observation group(94.29%)was significantly higher than that of control group(80.00%)(x2=9.104,P<0.05).Conclusion:The therapy of AIRVO2respiratory humidification apparatus combined with conventional treatment for patients of chronic obstructive pulmonary disease(COPD)complicated with type II respiratory failure can obtain more ideal effect,and it can obviously improve clinical indicators and has better safety.
