放射科住院医师规范化培训模式的探索与实践

The exploration and practice of standardized residency training in radiology

随着以患者为核心的多学科协作诊治模式逐步应用于临床,"以核心能力为导向的训练"已经成为毕业后医学教育的新理念。美国毕业后医学教育认证委员会将住院医师的核心能力概括为6个方面,即病患照顾能力、医学知识、临床工作中学习与提高、人际关系与沟通技巧、职业精神和在医疗系统中的执业能力。本研究针对我国放射科住院医师规范化培训的现状和存在的问题,并结合放射科住院医师自身成长的规律,将放射科住院医师的核心能力概括为:良好的职业道德和人际沟通能力;正确的放射科临床思维与工作方法,以及最基本的临床急救技能;独立从事放射科临床工作,并树立终生接受医学教育的观点;通过执业医师资格考试和大型仪器设备上岗证考试;具有一定的教学和科研能力。基于以上核心能力,本研究创新性地提出了放射科住院医师规范化培训"三层九级"模式,对新时期"5+3"模式下的放射科住院医师规范化培训提供借鉴和启示。

With the patient as the core of the multi-disciplinary collaborative diagnosis and treatment model gradually applied to clinical,core competencies based residency training is a new concept of graduate medical education.Accreditation Council for Graduate Medical Education(ACGME)proposed six core competencies,which are patient care,medical knowledge,systems-based practice,practice-based learning and improvement,professionalism,interpersonal and communication skills.Based on the status quo and existing problems of the standardized radiology residency training in China and radiology resident's own growth pattern,we conclude that the core competencies in standardized radiology residency training should include good professional ethics and interpersonal skills;logical clinical thinking and approach in radiology practice;basic skills to handle medical emergencies;ability to practice radiology independently;lifetime commitment to medical education;ability to pass specialty board examinations and required certifications,and to teach and conduct scientific research.Based on the above core competencies,this study creatively proposed the“three-stage,nine-level”model in radiology residency training.This model will provide references and enlightenment for the standardized radiology residency training in the new era of“5+3”model.
