摘要
目的:探讨云南省妇幼保健院产科门诊孕17周前孕产妇及其丈夫地中海贫血的携带情况,并对夫妇同型携带进行干预。方法:对2016年来本院门诊进行产检的300对夫妇静脉血用SYSMEX XT-1800全自动血细胞分析仪做血细胞分析,用SEBIA Capiliary 2进行血红蛋白电泳分析,对血红蛋白筛查阳性的夫妇进行基因诊断,对结果进行统计分析。结果:门诊产检的群体人口背景组成较复杂,包括了13个民族和19个籍贯,地中海贫血的血细胞筛查阳性率为11.00%,电泳的阳性率为7.50%,基因诊断阳性率为2.33%。检出的基因型别为αα/-α^(3.7),αα/--^(SEA),IVS-Ⅱ-654复合αα/-α^(4.2),CD17,CD41-42,βE,βE复合αα/-α^(3.7)共7种类型,其中以CD17和αα/--^(SEA)为主,没有发现地中海贫血同型夫妇。结论:了解了本院产科门诊的地中海贫血流行情况,为优生优育及地中海贫血的防控和诊断提供重要依据。
Objective:To investigate the carrying status of thalassemia in17-week pregnant women and their husbands in obstetric clinic of the MCH hospital,and to intervene in the same type of couples.Methods:Blood cell in venous blood of300outpatient couples in our hospital in2016was analyzed by the use of SYSMEX XT-1800automatic blood cell analyzer.SEBIA Capiliary2hemoglobin was analyzed;suspected hemoglobin couples were given a gene diagnosis,and the statistical analysis of the results was conducted.Results:Demographic composition of the outpatient group was relatively complex,including13ethnic groups and19birth places.The positive rate of blood cells in screening of thalassemia was11.00%,the positive rate of electrophoresis was7.50%,and the positive rate of genes diagnosis was2.33%.The positive rate of red blood cell parameters and screening of male and female was only HGB and HbA2measurement data test was P<0.05.The difference was not statistically significant with Genotype detection ofαα/-α3.7,αα/--SEA,IVS-II/-654compositeαα/-α4.2,CD17,CD41-42,βE,βE compositeαα/-α3.7total of seven genotype,in which the majority had CD17andαα/--SEA thalassemia.The same type of thalassemia couples was not found.Conclusion:It is significantly meaningful to analyze the epidemic of thalassemia in gynecology clinic pregnancy before17-week pregnancy,and to acquire the gynecology clinic diagnosis of epidemic situation,which can provide guidance for eugenics and prevention and diagnosis of thalassemia in our province.
