针灸结合补阳还五汤治疗气虚血瘀型中风的临床研究

Clinical study onthe type of blood stasis due to qi deficiency stroke treated by acupuncture and Buyang Huanwu decoction

目的探讨针灸结合补阳还五汤治疗气虚血瘀型中风的临床疗效。方法选取245例气虚血瘀型患者,随机分为两组,对照组患者采取常规西医治疗,治疗组在对照组基础上,服用补阳还五汤,同时采取中医针灸治疗。结果治疗组和对照组总有效率分别为96.29%、83.63%,差异有统计学意义(P<0.05);两组患者中医证候评分比较,两组患者治疗前后Barthel评分、NIHSS评分均有所改善,且治疗结束后治疗组Barthel评分和NIHSS评分较对照组均有统计学意义(P<0.05);两组患者治疗后生活和语言能力均明显改善(P<0.05),治疗组血脂三项均显著性降低(P<0.05),且血浆黏度、血小板聚积率及红细胞聚积指数均较治疗前显著改善(P<0.05),对照组血脂三项、血浆黏度、血小板聚积指数治疗前后变化比较差异无统计学意义。随访,治疗组复发8例(5.92%),对照组复发12例(10.90%),组间复发率比较差异具有统计学意义(P<0.05)。结论传统中医针灸结合经典中药方剂补阳还五汤治疗气虚血瘀型中风临疗效确切,可进一步进行临床推广应用。

Objective To explore the clinical effect of acupuncture combined with Buyang Huanwu Decoction in treating qi deficiency and blood stasis type stroke.Methods245patients with qi deficiency and blood stasis were randomly divided into two groups.The patients in the control group were treated with conventional western medicine.On the basis of the control group,the treatment group was treated with Buyang Huanwu Decoction.Results The total effective rate of the treatment group and the control group were96.29%and83.63%respectively,the difference was statistically significant(P<0.05).The scores of TCM syndrome were compared between the two groups before and after treatment,Barthel score and NIHSS score were(P<0.05).The scores of Barthel and NIHSS in the treatment group were significantly higher than those in the control group(P<0.05).The life and language ability of the two groups were significantly improved(P<0.05),and the plasma viscosity,platelet accumulation rate and erythrocyte accumulation index were significantly improved(P<0.05),the blood lipid of the control group,plasma viscosity and platelet aggregation index were not changed before and after treatment Obviously.There were8cases(5.92%)in the treatment group and12cases(10.90%)in the control group.There was significant difference between the two groups(P<0.05).Conclusion Traditional Chinese medicine acupuncture combined with classic Chinese medicine prescription Buyang Huanwu Decoction in the treatment of qi deficiency and blood stasis type stroke clinical efficacy,can be further clinical application.
