摘要 目的:观察Nd∶YAG激光消融术治疗玻璃体混浊的疗效。方法:回顾性分析2016-10/2017-01我科收治并确诊为玻璃体混浊并采用Nd∶YAG激光治疗的患者78例84眼的临床资料,采用χ~2检验、Mann-Whitney检验、KruskalWallis检验和Spearman检验等比较不同病因和玻璃体分级间的治疗效果。结果:治疗2wk后,所有患眼(84眼)中显效66眼(79%),有效16眼(19%),无效2眼(2%),激光治疗的总有效率为98%;不同病因引起的玻璃体混浊,其治疗效果和治疗次数差异没有统计学意义(H=3.842,P=0.146;χ2=0.070,P=0.966);玻璃体混浊Ⅰ级、Ⅱ级的治疗有效率均为100%,Ⅲ级为92%,Ⅳ级为83%,玻璃体混浊分级与疗效呈负相关(rs=-0.027,P=0.013)。Ⅰ级患眼需要治疗1次的有38眼(90%),Ⅱ级的有21眼(88%),Ⅲ级的有9眼(75%),Ⅳ级的有2眼(33%),不同分级的患眼治疗次数差异具有统计学意义(U=2.580,P=0.010)。结论:Nd∶YAG激光玻璃体消融术是治疗玻璃体混浊的一种操作简单、方便。 AIM:To observe the effect of Nd-YAG laser ablation on vitreous floaters.METHODS:From October2016to January2017,78cases(84eyes)with vitreous floaters treated with Nd-YAG laser ablation were involved.Chi-square test,Mann-Whitney Test,Kruskal-Wallis test and Spearman test were used to compare the therapeutic effect with different etiologies and grades.RESULTS:After two-week treatment,there were66eyes(79%)with a significant resolution,16eyes(19%)with an improvement and2eyes(2%)invalid.The total effective rate was98%.There was no significant difference between the therapeutic effect and times in different etiologies(H=3.842,P=0.146;^=0.070,P=0.966);the effective rate were100%in grade I and II,92%in grade III,83%in grade IV,and the grade had a negative correlation with the therapeutic effect(rs=-0.027,P=0.013).There were38eyes(90%)in grade I,21eyes(88%)in grade II,9eyes(75%)in grade III,and2eyes(33%)in grade IV needed once treatment.There was significant difference between different grades and treatment times(L/=2.580,P=0.010).CONCLUSION:Nd-YAG laser ablation is a simple and convenient operation for vitreous floaters,and could improve the symptom of vitreous floater rapidly.
