高原地区藏族正常人群甲状腺功能值参考范围的临床研究

Clinical Study on the Reference Range of Thyroid Function Value Among Normal Tibetan Population in Plateau Area

目的探讨高原地区藏族正常人群甲状腺功能数值,为诊断甲状腺疾病提供参考范围。方法选取居住在拉萨的藏族人群300例为研究组(男136例,女164例)。同时,选取居住在上海的300例汉族人群为对照组(男167例,女133例)。通过化学发光免疫法,测定甲状腺功能的各项数值(TT3,TT4,FT3,FT4,TSH),并进行统计学分析。结果(1)研究组的TSH值为(2.2±1.2m IU),对照组的TSH值为(1.9±0.9m IU/l),具有统计学差异(P<0.05);(2)研究组的TT3,TT4,FT3,FT4值分别为(89.4±18.0ng)、(5.2±1.6ng)、(2.4±0.38ng/ml)、(1.3±0.8ng),对照组中的TT3,TT4,FT3,FT4值分别为(88.3±8.9ng)、(5.1±1.3ng)、(2.3±0.4ng/ml)、(1.35±0.3ng),差异无统计学意义(P>0.05);(3)两组中男性的TT3,TT4,FT3,FT4值,无统计学差异(P>0.05);(4)两组中女性的TT3,TT4,FT3,FT4值,无统计学差异(P>0.05);(5)本研究中,67岁以上人群的T3,FT3值相对偏低,但无统计学差异。结论本研究中,高原地区藏族人群的甲状腺功能各项数值,可作为诊断甲状腺疾病的参考范围,但由于样本量较少,有待扩大样本数量进行深入研究。

To investigate the reference range of thyroid function value among normal Tibetan population in plateau area,in order to provide foundation for clinical diagnosis of thyroid disease.Methods:In our study,300Tibetans(136males and164females)living in Lhasa were selected as test group.In addition,300Han Chinese(167males and133females)living in Shanghai were selected as control group.The thyroid function value(TT3,TT4,FT3,FT4,TSH)were detected by Chemilu-minesent Immunoassay Assay,and the clinical data were analyzed.Results:(1)The value of TSH in test group(2.2±1.2mIU)was significantly higher than that in control group(1.9±0.9mIU/l)(P<0.05).(2)The value of TT3,TT4,FT3,FT4in test group were(89.4±18.0ng),(5.2±1.6ng),(2.4±0.38ng/ml),(1.3±0.8ng),respectively.The value of TT3,TT4,FT3,FT4in control group were(88.3±8.9ng),(5.1±1.3ng),(2.3±0.4ng/ml),(1.35±0.3ng),respectively.But no difference statistically between the two groups(P>0.05).(3)Within both groups,there were no significant differences in the value of TT3,TT4,FT3,FT4between males(P>0.05).(4)Within both groups,there were no significant differences in the value of TT3,TT4,FT3,FT4between females(P>0.05).(5)In our study,the value of T3,FT3were relatively low in adults aged over67years,but no statistics difference.Conclusions:The reference range of thyroid function value can provide foundation for clinical diagnosis of thyroid disease among Tibetans.However,further study is still needed because of the limited cases allotted.
