线性可调碳酸氢根离子浓度对纠正酸中毒的影响

摘要 目的:探讨线性调整B液电导度对透析液中缓冲剂及其他离子浓度变化的影响。方法:选取威海威高血液净化中心日机装系列DBB-27C血液透析机,经调校完毕后采用同一批次碳酸氢盐透析液进行模拟透析,在维持透析液电导度14 mS/cm的情况下,以0.2 mS/cm的波段调整B液电导度在2~4 mS/cm范围内波动,并抽取每一波段的透析液进行离子检测,比较缓冲剂及其他离子变化,期间采用血液透析机分析仪全程监控电导度变化,避免由于透析机电导度误差或透析液因素影响实验数据。结果:透析液浓度与电导度不呈线性相关,钠离子(Na^+)无明显变化,其他离子与B液电导度呈轻微反向相关,但在该范围内碳酸氢根离子(HCO_3^-)与B液电导度具有显著的依存关系,且在酸洗消毒充分的情况下,多次实验后未发现碳酸钙和碳酸镁沉淀现象。结论:透析液浓度与电导度不呈线性相关,故可尝试通过调整B浓缩液电导度纠正慢性肾功能衰竭引起的酸中毒。 Objective:To investigate the effects of electrical conductivity of linear adjustable B buffer for the changing of buffer agent and other ion concentration in dialysate.Methods:DBB-27C hemodialysis machine of Wego blood purification center diary installed series was selected to research the effect.The bicarbonate dialysate of same batch was adopted to simulate dialysis after adjustment was completed.Under the condition of maintaining the dialysate electrical conductivity was14mS/ cm,0.2mS/cm band was chose to adjust dialysate conductivity of B buffer and take it fluctuating in the range of2~4mS/cm.And then the dialysate of each band was extracted to detect ion concentration,and the changes of buffer and other ion were compared and analyzed.During this term,the hemodialysis machine analysis meter was adopted to monitor the change of dialysate conductivity so as to avoid the effect of dialysate conductivity error of hemodialysis machine or dialysate factors for experiment data.Results:There was no linear correlation between dialysate concentration and dialysate conductivity,and sodion was not obviously changed.And other ions was slight reverse correlation with conductivity of B butter.While there was a significantly dependence relationship between bicarbonate radial ion(HCO3-)and electrical conductivity of B buffer in the range,and under the condition of full sterilization by using acid pickling,the sediment phenomenon of calcium carbonate and magnesium carbonate didn't appear after many times experiments.Conclusion:There is no linear correlation between dialysate concentration and electrical conductivity.Therefore,try to adjust the electrical conductivity of B buffer might rectify acidosis caused by chronic renal failure.
