摘要
随着功能磁共振(functional MRI,f MRI)技术的发展,f MRI技术已经在神经外科得到了越来越广泛的应用,以保护脑功能进而提高手术预后。本综述对任务态f MRI、弥散张量成像(Diffusion Tensor Imaging,DTI)及静息态f MRI(rsf MRI)技术在神经外科手术计划中的应用进行了探讨。目前任务态f MRI与DTI纤维束追踪技术已经广泛应用于神经外科手术术前计划中,用以评估病灶与功能区的空间关系,具有较高的稳定性及可靠性。rs-f MRI相关研究也表明其在对于神经功能区、神经认知功能区、致痫灶的判断及定位方面的临床应用潜力。随着磁共振技术的不断完善,f MRI将进一步提高神经外科手术预后。
With the development of functional MRI(fMRI),it has been widely applied in neurosurgery to improve the surgical outcomes.This article reviewed the usage of task-based fMRI,diffusion tensor imaging(DTI)and resting-state fMRI(rs-fMRI)in neurosurgery.At present,task-based fMRI and DTI tractography has been used in the preoperative planning of the surgical corridor,quantifying the spatial relationship between lesion and functional brain areas,and evaluating surgical risks.Research about rs-fMRI elucidates its potential in the location of neurological function areas,neurocognitive areas and epileptogenic focus.FMRI will play more important role in improving the neurosurgical outcomes in the future.
