基于数据挖掘探讨中药熏洗法治疗脑卒中后肩手综合征的组方规律分析

Compatibility rules of Chinese medicinein treatment of shoulder hand syndrome based on data mining

目的:通过搜集中药外用治疗脑卒中后肩手综合征文献的处方用药,探索临床中药外用复方治疗脑卒中后肩手综合征的用药规律,为临床用药提供参考依据。方法:通过计算机检索,全面收集中药外治法治疗脑卒中后肩手综合征的随机对照试验(RCT),对文献进行收集、整理、归纳和分析,运用频数分析方法探究现代临床外用中药治疗卒中后肩手综合征的用药规律。结果:45首方共使用中药81种。使用频次前5位的药物依次为红花(33)、伸筋草(23)、桂枝(23)、制川乌(20)、透骨草(20)。基于改进的互信息法的药物间关联度前3位依次为制川乌、独活、桂枝、虎杖,络石藤、制草乌;基于复杂系统熵聚类的药物核心组合,再通过无监督的熵层次聚类算法,形成6首中药熏洗治疗卒中后肩手综合征新处方。结论:卒中后肩手综合征熏洗药物主要配伍是活血化瘀、温经通脉,祛风除湿、和血通痹及消肿止痛,且以辛、苦、温味药物为主。

Objective:To investigate the clinical prescription of external use of a compound traditional Chinese medicine for the treatment of shoulder-hand syndrome after stroke through exploring prescriptions of external use of traditional Chinese medicines for shoulder-hand syndrome after stroke in literature,so as to provide reference for clinical medication.Methods:We collected of all randomize controlled trials,on external treatment of traditional chinese medicine for shoulder hand syndrome,by searching electronic bibliographic databases,and then used association rules to analyze the prescription compatibility law.Result:81kinds of traditional Chinese medicine were refered to in45prescriptions.Medicines with the top5high frequency of usage are safflower(33),lycopodium clavatum(23),cassia twig(23),radix aconite preparata(20)and tuberculate speranskia herb(20),successively.Based on modified mutual information,medicines with the top3high correlation are radix aconite preparata and radix angelicae tuhuo,cassia twig and polygonum cuspidatum,and Chinese starjasmine and radix aconiti kusnezoffii preparata.According to core medicine combination based on complex system entropy cluster algorithm,a new prescription including6traditional Chinese medicines for the fumigation and washing therapy of shoulder-hand syndrome after stroke was formed using unsupervised entropy hierarchical clustering algorithm.Conclusion:For shoulder-hand syndrome,the main compatibility of traditional Chinese medicine with blood-activating and stasis-resolving medicinals,Wenjingtongmai,eliminating wind and ampness,hexuetongbi,and reduce swelling and ease pain,and the tastes of drugs were mainly spicy,bitter and warm.
