超声造影对前列腺脓肿精准介入治疗的价值

Value of contrast-enhanced ultrasound in accurate interventional treatment of prostatic abscess

目的评价超声造影下经直肠前列腺脓肿精准介入治疗的临床应用价值。方法对4例前列腺脓肿的5个病灶进行超声造影下介入穿刺,穿刺操作均在30~60 s之间完成。抽吸完脓液后脓腔内反复用甲硝唑溶液冲洗后留液2~10 m L。术后1、3个月作超声造影评价治疗效果。结果 5个液化脓腔均准确穿刺,并抽出黏稠脓液3~15 m L。3个月后彩超造影均未见脓肿复发,前列腺组织呈均匀等增强。结论超声造影引导经直肠前列腺穿刺,可精准进入前列腺脓肿进行治疗,并有利于术后评估,值得临床推广。

ObjectiveTo evaluate the clinical value of interventional therapy in the treatment of benign prostatic abscess by ultrasound contrast.MethodsFour cases of benign prostatic abscess with5lesions were treated by ultrasound imaging and interventional procedures between30and60seconds.The pus cavity was cleared by repeated suction with metronidazole rinse and retained about2to10mL.The therapeutic effect was evaluated by contrast enhanced ultrasound at3months after operation.ResultsFive gas pus cavities were punctured accurately and respective3to15mL of sticky pus had been drawn.One month and three months after interventional therapy,no complications were found and prostate tissues were homogeneously enhanced.ConclusionsPreoperative application of contrast enhanced ultrasound can make detailed and accurate operation plan,its intraoperative application can achieve accurate guidance of puncture,and postoperative application can help the evaluation of therapeutic effect,which is worthy of clinical application.
