外周血T-SPOT.TB与胸腔积液ADA活性检测对结核性胸腔积液的诊断价值 被引量:5

摘要 目的探讨外周血结核感染T细胞斑点试验(T-SPOT.TB)与胸腔积液腺苷脱氨酶(ADA)活性单独或联合检测对结核性胸腔积液的诊断价值。方法选取结核性胸腔积液患者110例作为结核组、非结核性胸腔积液患者60例作为非结核组。两组均于治疗前采集外周静脉血进行T-SPOT.TB,计数斑点形成细胞数;采集胸腔积液进行ADA活性检测。根据外周血T-SPOT.TB检测的斑点形成细胞数及ADA活性绘制ROC曲线,比较二者单独检测或联合检测对结核性胸腔积液的诊断效能。结果结核组外周血T-SPOT.TB阳性率、胸腔积液ADA阳性率高于非结核组(P<0.05);结核组外周血T-SPOT.TB阳性率高于胸腔积液ADA阳性率(P<0.05)。两组外周血T-SPOT.TB的A、B孔斑点形成细胞数及胸腔积液ADA活性比较差异有统计学意义(P<0.05)。结核组外周血T-SPOT.T的ROC曲线下面积大于胸腔积液ADA活性检测(P<0.05),其敏感度高于胸腔积液ADA活性检测(P<0.05)。二者联合检测诊断结核性胸腔积液的敏感度高于单独检测(P均<0.05)。结论外周血T-SPOT.TB与胸腔积液ADA活性联合检测对结核性胸腔积液的诊断效能高于二者单独检测。
