异种脱细胞真皮基质敷料在小儿Ⅱ度烧伤中的应用

Application of xenogenic acellular dermal matrix dressing in children with second degree burn

目的观察异种脱细胞真皮基质敷料应用于小儿Ⅱ度烧伤创面的治疗效果。方法选取2016年9月-2017年9月共计100例烧伤患儿,烧伤深度为浅Ⅱ度和深Ⅱ度,观察组接受清创脱细胞真皮基质覆盖术的患儿共54例(其中浅Ⅱ度34例,深Ⅱ度20例)。对照组采用常规换药治疗的患儿共46例(其中浅Ⅱ度30例,深Ⅱ度16例)。分析比较两组患儿的创面愈合情况及瘢痕发生率。结果观察组浅、深Ⅱ度创面愈合时间分别为(8.18±2.08)d、(19.25±3.11)d,换药次数分别为(2.41±0.78)、(6.05±1.28),其中深Ⅱ度创面的植皮率为30.0%,瘢痕发生率为50.0%。对照组浅、深Ⅱ度创面愈合时间分别为(10.43±2.66)d、(21.31±2.98)d,换药次数分别为(4.37±1.07)、(8.94±1.06),其中深Ⅱ度创面的植皮率为43.8%,瘢痕发生率为87.5%。观察组的创面愈合时间及换药次数明显低于对照组(P<0.01),观察组中的深Ⅱ度创面植皮率及后期瘢痕发生率均低于对照组。结论异种脱细胞真皮基质敷料应用于小儿Ⅱ度烧伤能有效缩短创面愈合时间,减少换药次数,促进创面愈合,减少瘢痕发生率。

Objective Observe the therapeutic effect of xenogenic acellular dermal matrix dressing on pediatric degreeⅡburn wounds.Methods Selected100cases of the burned children during September2016-September2017;The severity of the burn was ShallowⅡand deepⅡdegrees;children with accepting the dermal matrix coverage of debridement had54cases(Among them,34cases for ShallowⅡdegrees,and20cases for DeepⅡdegrees),as the observation group.There were46cases children with accepting conventional dressing treatment.(Among them,30cases for ShallowⅡdegrees,and16cases for DeepⅡdegrees),for the control group.Analyzed and contrasted the situation of the wound healing of two groups of children and the incidence rate of scar.Results The healing time of shallow and deepⅡwounds in the observation group were(8.18±2.08)d and(19.25±3.11)d,and the time of dressing frequency is(2.41±0.78),(6.05±1.28),the skin graft rate of deepⅡdegree wound was30.0%,incidence of scar was50.0%.In the control group,the healing time of shallow and deep second degree wounds was(10.43±2.66)d,(21.31±2.98)d,and the times of dressing change were(4.37±1.07)and(8.94±1.06)respectively,among deepⅡdegree wound,skin graft rate was43.8%,the incidence of scar was87.5%.The wound healing time and dressing frequency in the observation group were significantly lower than those in the control group(P<0.01),in the observation group,the skin grafting rate of deepⅡdegree wounds and the incidence of post-scarring were lower than those of the control group.Conclusion Heterogeneous decellularized dermal matrix dressing applied toⅡdegree burns can effectively shorten the wound healing time,reduce the dressing frequency,promote wound healing and reduce scar incidence rate.
