低位PPH治疗炎性水肿型混合痔的对比研究

A comparative study on treating inflammatory edema of mixed hemorrhoids by low PPH

目的:探究炎性水肿型混合痔患者在使用低位PPH治疗后效果。方法:研究对象为2016年1月-2016年7月收入200例炎性水肿型混合痔患者,用随机排列表法分成试验组与对照组,100例采用低位PPH术治疗作为试验组,另100例采用常规PPH联合外剥内扎术(CG)治疗作为对照组。比较两组患者实施不同治疗在手术、住院时间、费用、并发症发生率及随访1年后术后外痔脱出、曲张静脉痔核形成率、术后复发率、患者满意度影响。结果:试验组总有效率(99.00%)比对照组(93.00%)高(χ~2=4.688,P<0.05);在手术、住院时间及住院费用上比对照组少(t=-36.422、-17.437、-12.155,P<0.05);在并发症发生率上(2.00%)比对照组(10.00%)低(χ~2=4.344,P<0.05);在外痔脱出率上(1.00%)比对照组(7.00%)低(χ~2=4.688,P<0.05);在曲张静脉痔核形成率上(0.00%)比对照组(6.00%)低(χ~2=6.186,P<0.05);在术后复发率上(0.00%)比对照组(5.00%)低(χ~2=5.128,P<0.05);且治疗满意度上(99.00%)比对照组(89.00%)高(χ~2=8.865,P<0.05)。结论:炎性水肿型混合痔患者采用低位PPH治疗,不仅会降低术后住院时间、治疗费用,还能减少并发症及复发,提升治疗效果,此方法值得应用与推广。

Objective:To explore the effect of low PPH on inflammatory edema of mixed hemorrhoids.Methods:200inflammatory edema of mixed hemorrhoids patients in our hospital from January2016to July2016were selected as the research subjects and randomly divided into the experimental group and the control group.100cases who were treated with low PPH were selected as the experimental group,the other100cases who were treated with conventional PPH plus Milligan Morgan(CG)were selected as the control group.After different treatment,operation,hospitalization time,cost,complication rate and the external hemorrhoids prolapse after1year of postoperative follow-up,varicose vein formation rate,postoperative recurrence rate and patients’satisfaction were compared.Results:The total effective rate in the experimental group(99.00%)was higher than that in the control group(93.00%)(χ2=4.688,P<0.05).The operation,hospitalization time and cost in the experimental group was less than that in the control group(t=-36.422,-17.437,-12.155,P<0.05).The complication rate in the experimental group(2.00%)was lower than that in the control group(10.00%)(χ2=4.344,P<0.05).The external hemorrhoids prolapse rate in the experimental group(1.00%)was lower than that in the control group(7.00%)(χ2=4.688,P<0.05).The varicose vein formation rate in the experimental group(0.00%)was lower than that in the control group(6.00%)(χ2=6.186,P<0.05).The postoperative recurrence rate in the experimental group(0.00%)was lower than that in the control group(5.00%)(χ2=5.128,P<0.05).The satisfaction rate in the experimental group(99.00%)was higher than that in the control group(89.00%)(χ2=8.865,P<0.05).Conclusion:Low PPH on inflammatory edema of mixed hemorrhoids could reduce the postoperative hospitalization time,cost of treatment,complications,improve the therapeutic effect,and is worthy of clinical application and promotion.
