不同病期非霍奇金淋巴瘤患者治疗前后血浆VEGF浓度变化及临床病理特征分析

P-VEGF concentration variation and different clinical and pathological features of the disease before and after treatment of patients with NHL

目的对比分析不同病期非霍奇金淋巴瘤(NHL)患者治疗前后血浆血管内皮生长因子(P-VEGF)的浓度变化及临床病理特征。方法选择2013年5月至2016年5月经组织病理学检查确诊并住院治疗的NHL患者200例为实验组,同期200例进行体检的健康志愿者为对照组进行回顾性研究。实验组用异环磷酰胺、强的松、长春新碱、吡喃阿霉素联合化疗,前3 d静脉注射吡喃阿霉素;前5 d静滴异环磷酰胺,每天20 g,需加500 ml生理盐水;为预防泌尿道毒性反应,同时静注美斯钠注射液,其剂量为异环磷酰胺的20%。对比分析P-VEGF的浓度在不同病期治疗前后变化及与临床病理特征的关系。结果实验组患者治疗前P-VEGF浓度(178.5±13.67)pg/ml明显高于完全缓解者(69.6±6.43)pg/ml,差异有统计学意义(P<0.05);完全缓解者的P-VEGF浓度(69.6±6.43)pg/ml明显低于部分缓解者(105.5±10.28)pg/ml,差异有统计学意义(P<0.05),与复发进展者相比,明显降低,差异有统计学意义(P<0.05);对照组的P-VEGF浓度(64.3±7.91)pg/ml与完全缓解者相比差异无统计学意义(P>0.05);高浓度的P-VEGF患者的血清乳酸脱氢酶(LDH)水平高于正常值,差异有统计学意义(P<0.05)。结论 NHL患者在首次治疗前PVEGF浓度较高,经首次化疗后,完全缓解者自身浓度回归正常,随着自身复发病情的进展,浓度再次升高。P-VEGF浓度与病理特征中的血清LDH水平有关。

ObjectiveThe analyze P-VEGF concentration variation and different clinical and pathological features of the disease before and after treatment of patients with NHL.Methods200patients with NHL were diagnosed and treated by histopathological examination from May2013to2016in the hospital.200healthy volunteers were retrospectively studied in the same period.The experimental group was treated with ifosfamide,prednisone,vincristine and pirarubicin combined with chemotherapy,the first three days of intravenous injection of pirarubicin;the first five days of intravenous infusion of ifosfamide,each for the prevention of urinary Road toxic reaction,while intravenous Meisner,the dose of isosorbide20%.The changes of P-VEGF concentration before and after treatment were compared with clinicopathological features.ResultsThe concentration of P-VEGF(178.5±13.67)pg/ml in the experimental group was significantly higher than that in the complete remission(69.6±6.43)pg/ml,the difference was statistically significant(P<0.05),the complete remission and partly remission(105.5±10.28)pg/ml had significant difference(P<0.05),and were higher than those in progression and recurrence ones(P<0.05).There was no significant difference in the P-VEGF concentration(64.3±7.91)pg/ml between the control group and the complete remission group(P>0.05).ConclusionThe serum levels of H-desensitization(LDH)is higher before treatment.The concentration of P-VEGF is related to the serum LDH.
