血清25-(OH)D_3水平与胃癌术后3年复发、转移及生存率的相关性

Correlation of serum 25-(OH)D_3 level with recurrence, metastasis and survival rate of 3 years after gastric cancer surgery

目的研究血清25-(OH)D_3水平与胃癌术后3年复发、转移及生存率的相关性。方法研究对象为70例在我院接受胃癌根治术的患者,随访3年,记录复发、转移及患者生存率,比较复发、转移及死亡等不同临床结局患者间25-(OH)D_3水平差异,分析25-(OH)D_3与患者复发、转移及生存率的关系。结果平均随访(24.37±8.12)月,随访期间复发16例,淋巴结或远处转移12例,死亡15例,失访或意外死亡3例,存活52例,存活率74.29%。复发转移患者25-(OH)D_3水平显著低于未复发转移者(P<0.05)。死亡患者25-(OH)D_3水平显著低于存活者(P<0.05)。不同临床分期和病理分型间25-(OH)D_3水平差异显著(P<0.05),随着临床分期和病理分型进展,25-(OH)D_3水平逐渐降低(P<0.05)。结论血清25-(OH)D_3水平与胃癌进展密切相关,高水平25-(OH)D_3对胃癌进展具有抑制作用,而低水平25-(OH)D_3多提示预后不良。

Objective To explore the correlation of serum25-(OH)D3level with recurrence,metastasis and survival rate of3years after gastric cancer.Methods Seventy patients with gastric cancer underwent radical gastrectomy were enrolled in this study.Follow up3years,the recurrence,metastasis,and patient's survival rate were recorded,the difference of25-(OH)D3level between patients with different clinical outcomes was compared.The relationship between25-(OH)D3and the recurrence,metastasis and survival rates of patients were analyzed.Results The average follow-up time was(24.37±8.12)months.During the follow-up period,there were16cases of recurrence,12cases of lymph node or distant metastasis,15cases of death,3cases of lost or accidental death,52cases of survival.The survival rate was74.29%.The levels of25-(OH)D3in patients with recurrence and metastasis were significantly lower than those without recurrence(P<0.05).The25-(OH)D3level in death cases were significantly lower than that in the survived cases(P<0.05).There was a significant difference in25-(OH)D3levels among patients with different clinical stages and pathological types(P<0.05).The level of25-(OH)D3gradually decreased with the progression of clinical stage and pathologicalcl assification(P<0.05).Conclusion The level of serum25-(OH)D3is closely related to the progression of gastric cancer.High level25-(OH)D3has an inhibitory effect on the progression of gastric cancer,while low level of25-(OH)D3may indicate poor prognosis.
