在麻醉恢复室设立加强监护室的实践效果研究

Effects of establishing anesthesia intensive care unit in post-anesthesia care unit

目的探讨在麻醉恢复室(PACU)设立加强监护单元对麻醉医疗质量的影响。方法通过从麻醉加强监护单元(AICU)建立的条件、管理方面探索实施的效果。其中AICU的建立条件包括设立的位置、仪器与设备、人员设置要求、收治对象及工作流程和患者转出标准等方面;AICU的管理主要从制度化管理、加强医院感染管理、实行轮岗制及加强人员培训方面进行。结果 AICU自2013年设立4年来收治各类患者4830例,除10例患者因持续低血压、脉搏血氧饱和度低及外科并发症等因素转入ICU继续治疗外,其余患者均安全返回普通病房,在AICU平均驻留时间(6.57±1.48)h。结论建立AICU能明显改善麻醉后对患者的安全监护,有效降低了围术期麻醉并发症的发生率和死亡率。

Objective To explore the effects of setting an anesthesia intensive care unit(AICU)in post-anesthesia care unit(PACU).Methods To set up a special anesthesia intensive care unit for the patients who need extra cares in PACU,the location and the equipment are modified;the standards of nurse training and patient care are adjusted;and the new protocols of the treatment and the transfer-out from PACU are established.Results There were4830patients received treatments in AICU during the last three years.Except10patients were sent to intensive care unit(ICU),the others of4820patients were safely transferred to the surgical ward.Conclusion To set an AICU in PACU can obviously improve the safety and quality of patient care after anesthesia,and effectively reduce the transferring rate from PACU to ICU.
