摘要
目的:探讨乌司他丁联合奥曲肽治疗对重症胰腺炎患者全身炎症反应、肠黏膜功能的影响。方法:选择我院收治的82例重症胰腺炎患者,回顾治疗方案,并分为对照组43例、乌司他丁组39例,对照组接受常规+奥曲肽治疗,乌司他丁组接受常规+奥曲肽+乌司他丁治疗,均持续1周。对比两组患者治疗前后全身炎症反应、肠黏膜功能的差异。结果:治疗前,两组血清中炎症介质、肠黏膜功能指标含量的差异无统计学意义(P>0.05)。治疗1周后,乌司他丁组患者血清中炎症介质IL-1β、IL-8、HMGB-1、MCP-1的含量以及肠黏膜功能指标ET、DAO、D-lactose的含量低于对照组(P<0.05)。结论:重症胰腺炎患者在常规治疗基础上加入乌司他丁联合奥曲肽治疗,可进一步抑制机体全身炎症反应同时积极保护肠黏膜功能。
Objective:To investigate the effects of ulinastatin combined with octreotide therapy on systemic inflammatory response and intestinal mucosal function in patients with severe pancreatitis.Methods:82patients with severe pancreatitis who were treated in our Hospital between March2015and August2017were reviewed and divided into control group(n=43)and ulinastatin group(n=39),control group received conventional+octreotide therapy,ulinastatin group received conventional+octreotide+ulinastatin therapy,and both groups were treated for1week.The differences in systemic inflammatory response and intestinal mucosal function were compared between the two groups before and after treatment.Results:Before treatment,the differences in serum levels of inflammatory mediators and intestinal mucosal function indexes were not statistically significant between the two groups.After1week of treatment,serum levels of inflammatory mediators IL-1β,IL-8,HMGB-1and MCP-1as well as intestinal mucosal function indexes ET,DAO and D-lactose of ulinastatin group were lower than those of control group.Conclusion:Ulinastatin combined with octreotide therapy on the basis of conventional treatment can further inhibit the systemic inflammatory response and actively protect the intestinal mucosal function in patients with severe pancreatitis.
