摘要
目的分析研讨腹腔镜肾部分切除术和开放手术治疗肾肿瘤对患者炎症指标、前白蛋白及肾功能的影响。方法回顾性分析2015年3月至2016年9月期间86例肾肿瘤接受肾部分切除术的患者,根据手术方式不同分为研究组和对照组。研究组接受腹腔镜肾部分切除术治疗,对照组接受开放肾部分切除术治疗,监测患者手术前后相关指标,如白细胞计数、C反应蛋白(CRP)、前白蛋白及血肌酐,并对比分析。结果两组术后白细胞计数、血肌酐较术前升高,前白蛋白较术前降低(P<0.001);研究组术后的白细胞计数、CRP、血肌酐低于对照组,前白蛋白高于对照组,差异有统计学意义(P<0.001)。结论在条件允许的情况下,临床治疗肾肿瘤可考虑行腹腔镜肾部分切除术,有利于降低手术创伤,减轻炎性反应,对机体营养水平、组织修复及肾功能的影响较小。
Objective To analyze the effects of laparoscopic partial nephrectomy and open partial nephrectomy on inflammatory parameters,prealbumin and renal function.Methods A retrospective analysis was performed in86patients with renal tumors undergoing partial nephrectomy between March,2015and September,2016.The patients were divided into study group and control group according to different surgical procedures.The study group received laparoscopic partial nephrectomy,and the control group received open partial nephrectomy.Then,related indexes before and after operation,such as white blood cell count,C reactive protein(CRP),prealbumin and serum creatinine were monitored,compared and analyzed between the two groups.Results The white blood cell count and serum creatinine in the two groups were higher than those before operation,and the prealbumin was lower than that before operation(P<0.001).The white blood cell count,CRP and serum creatinine in the study group were lower than those in the control group,and the prealbumin was higher than that in the control group,difference was statistically significant(P<0.001).Conclusion If conditions permit,laparoscopic nephrectomy may be considered in the clinical treatment of renal tumors,which is beneficial to reduce surgical trauma,relieve inflammatory response,and has less influence on the body nutrition level,tissue repair and renal function.
