联合护理干预对喉癌患者术后生活质量的影响

The effectiveness of combined nursing intervention on post-operation life quality of patients with laryngeal carcinoma

目的评价联合护理干预对喉癌患者术后生活质量的影响,以提升护理人员的专业水平。方法回顾性分析2015-11至2017-01吉林大学第一医院耳鼻喉头颈外科收治的接受手术治疗的158例喉癌患者的临床资料,按护理方法不同将其分为两组,其中2015-11至2016-05收治的接受喉癌手术患者为对照组(n=79),接受常规护理干预;2016-06至2017-01收治的接受喉癌手术患者为观察组(n=79),接受联合护理干预。分别在护理干预前后采用汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)、汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)评估两组患者的心理健康状况。采用中国头颈部癌生命质量量表(the Chinese quality of life instruments for cancer patients-head and neck cancer,QLICP-HN)评估两组护理干预后的生活质量。结果 (1)心理健康状况评分比较:经护理干预后,两组的HAMA、HAMD评分均显著低于护理前(P<0.05),且护理后观察组的各项评分均显著低于对照组(t=9.325,P<0.001;t=7.646,P<0.001)。(2)生活质量评分比较:经过护理干预后,观察组总分(424.73±10.02)分显著高于对照组的(325.56±9.88)分,差异具有统计学意义(t=-62.639,P<0.001),且观察组生活质量评分、躯体功能评分、心理功能评分、社会功能评分、共性症状及不良反应评分、特异模块评分均显著高于对照组(P<0.05)。结论对喉癌术后患者采取联合护理干预能有效改善患者的负性心理,提高其生活质量,缓解患者的不适症状,促进其身体恢复。

Objective The objective of this study was to evaluate the influence of combined nursing intervention on the life quality of post-operation patients with laryngeal carcinoma,and to improve the professional level of nursing staff.Methods A total of158cases of laryngeal carcinoma patients treated in the First Hospital of Jilin University were selected and divided into two groups based on the different nursing methods.Patients admitted between November2015and May2016were selected as the control group(n=79)and received routine nursing intervention;patients who were treated between June2016and January2017were selected as the observation group(n=79)and were provided with combined nursing intervention.The mental health status of the two groups was respectively evaluated by Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)before and after the intervention.The quality of life of the two groups after the nursing intervention was assessed using the Chinese quality of life instruments for cancer patients-head and neck cancer(QLICP-HN).Results(1)Psychological health status scores:after nursing intervention,the scores of HAMA and HAMD in the two groups were decreased compared to those prior to nursing(P<0.05),and the score of the observation group was significantly lower than that of the control group(t=9.325,P<0.001;t=7.646,P<0.001).(2)Quality of life scores:after nursing intervention,the total score of the observation group(424.73±10.02)was significantly higher than that of the control group(325.56±9.88),and the difference was statistically significant(t=-62.639,P<0.001).The quality of life,physical function,psychological function,social function,general symptoms,adverse reactions and specific module scores of the observation group were significantly higher than those of the control group(P<0.05).Conclusions Combined nursing intervention can improve the the psychological condition and quality of life,relieve the symptoms,and promote the recovery of post-operation patients with laryngeal carcinoma.
