结直肠癌患者临床分期与CEA和CA19-9的相关性分析

Correlations of serum CEA and CA19-9 levels with clinical staging of colorectal cancer

目的了解不同TNM分期的结直肠癌患者术前血清肿瘤标志物癌胚抗原(CEA)和糖类抗原19-9(CA19-9)水平。方法纳入350例不同TNM分期的结直肠癌患者,收集患者年龄、性别、肿瘤分化程度等病历资料,采用化学发光仪检测患者术前血清CEA和CA19-9水平,采用有序Logistic回归模型分析TNM分期与血清CEA、CA19-9的相关性。结果Ⅰ~Ⅳ期结直肠癌患者血清CEA阳性率分别为25.00%、36.69%、50.78%和66.67%,血清CA19-9阳性率分别2.94%、10.07%、17.97%和53.33%,血清CEA和CA19-9阳性率均随结直肠癌TNM分期进展呈升高趋势(P<0.05)。有序Logistic回归结果显示,血清CEA阳性(OR=1.790,95%CI:1.163~2.755)和CA19-9阳性(OR=3.476,95%CI:1.790~6.749)是结直肠癌临床分期进展的危险因素。结论血清CEA和CA19-9指标阳性与结直肠癌TNM分期进展有关,血清CEA和CA19-9水平可作为评估结直肠癌进展的辅助诊断指标。

Objective To evaluate the value of serum CEA and CA19-9concentration for clinical staging of colorectal cancer.Methods A total of350patients who underwent the surgical treatments for colorectal cancer between February2015to January2017were enrolled.The serum CEA and CA19-9were detected by chemoluminescence method.Results The positive rate of CEA of patients in stageⅠtoⅣwas25.00%,36.69%,50.78%and66.67%,respectively.The positive rate of CA19-9of patients in stageⅠtoⅣwas2.94%,10.07%,17.97%and53.33%,respectively.The positive rates of CEA and CA19-9were gradually increased with the stage developing(P<0.05).Results from multivariate logistic regression analysis showed that the positive levels of CEA and CA19-9were risk factors in the TNM staging of colorectal cancer.The ORs and95%CI were1.790(1.163-2.755)and3.476(1.790-6.749),respectively.Conclusion The positive serum concentrations of CEA and CA19-9showed significant associations with TNM staging.Preoperative serum concentrations of CEA and CA19-9could be auxiliary diagnostic indicators to assess the condition of colorectal cancer.
