血浆IL-33与冠状动脉病变程度的相关性分析

摘要 目的探讨冠心病患者血浆白细胞介素33(IL-33)水平与冠状动脉病变严重程度的关系。方法收集83例经冠状动脉造影确诊的冠心病患者,分为急性心肌梗死组(AMI组)17例,不稳定型心绞痛组(UAP组)23例,稳定型心绞痛组(SAP组)43例;30例冠状动脉造影正常者为对照组。采用酶联免疫吸附法测定其血浆IL-33水平。结果 (1)血浆IL-33浓度在AMI组、UAP组中较SAP组、对照组显著降低(P<0.001);(2)IL-33水平在Gensini积分小于20分组水平高于Gensini积分位于20~40分及大于40分(P<0.05),在单支、双支、三支病变组对比,差异无统计学意义(P>0.05);(3)Logistic回归分析发现,血浆IL-33浓度降低是冠状动脉病变和斑块不稳定的独立危险因素(OR=0.706,95%CI=0.482~1.052,P=0.024)。结论白细胞介素33与冠状动脉病变严重程度和斑块稳定性相关,可作为预测斑块稳定性的标志物。 Objective To investigate the relationship between serum level of IL-33and the severity of coronary lesions in patients with coronary heart disease.Methods83patients with CHD were divided into the stable angina pectoris group(n=43),the acute myocardial infarction group(n=17)and the unstable angina pectoris group(n=23)by coronary angiography,30normal subjects without CHD served as the control group.The serum level of IL-33was measured by enzyme linked immunosorbent assay.Results(1)The mean level of serum IL-33was obviously higher in the SAP and control group than in the AMI and UAP group(P<0.001);(2)Serum level of IL-33in groups with Gensini Score20~40and higher than40were lower than in group with Gensini Score lower than20(P<0.05);there were no significant differences of serum level of IL-33among the single vessellesion group,the double vessel lesion group and three vessel lesion group(P>0.05).(3)Logistic analysis indicated that the lower level of IL-33was an independent risk factor for coronary disease and plaque instability(OR=0.706,95%CI=0.482~1.052,P=0.024).Conclusion IL-33plays a role in the occurrence and development of athermanous plaque and acts as one of the biomarkers of vulnerable plaques.
