关节镜下自体和同种异体肌腱重建前交叉韧带的疗效比较

Arthroscopic Reconstruction of Anterior Cruciate Ligament With Autologous and Allogeneic Tendons

目的对比分析关节镜下自体肌腱重建前交叉韧带和关节镜下同种异体肌腱重建前交叉韧带的临床疗效。方法选取2016年5月—2017年10月来我院治疗的前交叉韧带损伤的患者110例作为研究对象,将其随机分为自体组(n=55)和异体组(n=55)。自体组患者采取应用自体肌腱实施关节镜下前交叉韧带重建,异体组患者采取同种异体肌腱实施关节镜下前交叉韧带重建。结果自体组患者的手术时间,明显比异体组长,差异具有统计学意义(P<0.05);自体组患者能术后发热时间,明显比异体组短,差异具有统计学意义(P<0.05);自体组和异体组患者的术后住院时间对比,差异无统计学意义(P>0.05)。两组患者治疗前的IKDC评分和Lysholm评分对比,差异无统计学意义(P>0.05);治疗6个月后的IKDC评分与Lysholm评分均较治疗前有显著提高,但两组提高程度均相当,差异无统计学意义(P>0.05)。结论关节镜下自体肌腱重建前交叉韧带和关节镜下同种异体肌腱重建前交叉韧带治疗前交叉韧带损伤,能够取得相当的临床治疗效果。

Objective To compare the clinical efficacy of arthroscopic reconstruction of anterior cruciate ligament with arthroscopic reconstruction of anterior cruciate ligament with allogeneic tendon.Methods110cases of anterior cruciate ligament injury treated in our hospital from May2016to October2017were selected as the study subjects.They were randomly divided into autologous group(n=55)and allogenic group(n=55).Autogenous tendons were used to reconstruct the anterior cruciate ligament under arthroscopy.Allogeneic tendons were used for the reconstruction of anterior cruciate ligament under arthroscopy.Results The operative time of the patients in the autologous group was significantly higher than that of the allogenic group(P<0.05).The time of postoperative fever in the patients with autologous group was significantly shorter than that of the allogeneic group(P<0.05).There was no significant difference in the time of postoperative hospitalization between the autologous group and the allogeneic group(P>0.05).There was no significant difference in the IKDC score and Lysholm score before treatment in the two groups(P>0.05).The IKDC score and Lysholm score after treatment for6months increased significantly compared with those before treatment,but the improvement degree of the two groups was all the same,and the difference was not statistically significant(P>0.05).Conclusion Arthroscopic reconstruction of anterior cruciate ligament with autologous tendon and arthroscopic reconstruction of anterior cruciate ligament with allogeneic tendon can achieve considerable clinical effect.
