骨水泥强化治疗老年退行性腰椎疾病的疗效研究 被引量:2

摘要 目的探讨骨水泥强化与常规椎弓根螺钉固定治疗老年退行性腰椎疾病的疗效。方法选取2010年4月-2015年6月于该院收治的老年退行性腰椎疾病患者80例,依据随机数表法分为强化组(骨水泥强化椎弓根螺钉固定)和常规组(常规椎弓根螺钉固定),比较两组患者的临床疗效、日本矫形外科学会腰痛评分(JOA)、视觉模拟评分(VAS)及影像学指标。结果强化组优良率(90.00%)高于常规组(70.00%)(P<0.05);两组患者的治疗后JOA、VAS评分有差异(P<0.05),两组患者治疗后椎间隙高度、椎体变形指数、固定节段Cobb角度及骨密度有差异(P<0.05),强化组均优于常规组。强化组融合率高于常规组,内固定松动率低于常规组。结论骨水泥强化椎弓根螺钉固定治疗老年退行性腰椎疾病成效显著,能够改善患者腰椎功能,缓解腰椎疼痛。 Objective To investigate the effect of bone cement augmentation and conventional pedicle screw fixation in the treatment of degenerative lumbar spine disease in the elderly.Methods Eighty elderly patients with degenerative lumbar disease treated in our hospital from April2010to June2015were selected.According to the random number table method they were divided into intensive group(bone cement augmentation pedicle screw fixation)and conventional group(conventional pedicle screw fixation).Clinical efficacy,JOA score,VAS score and radiographic parameters of the two groups were compared.Results The excellent rate of the intensive group(90.00%)was higher than that of the conventional group(70.00%,P<0.05).JOA and VAS scores of the two groupswere significantly different after treatment(P<0.05).The imaging indexes(intervertebral height,vertebral body deformation index,Cobb angle of fixed segment and bone mineral density)of the intensive group were better than those of the conventional group after treatment(P<0.05).In addition,the fusion rate of the intensive group was higher than that of the conventional group,while the loosening rate of the internal fixation was lower than that of the conventional group(P<0.05).Conclusions The bone cement augmentation pedicle screw fixation has remarkable effect in the treatment of degenerative lumbar disease,can significantly improve the function of lumbar vertebrae and relieve lumbar pain.
