摘要
目的探讨生脉注射液配合常规治疗改善AMI后心源性休克的疗效及对心功能、血清cTnI、cTnI-mRNA、CaM和尿量的影响。方法选择我院收治的AMI后心源性休克患者120例,随机分为观察组和对照组,各60例。对照组予以常规治疗,观察组在对照组的基础上予以生脉注射液。比较两组临床治疗效果,心功能指标(SBP、LVEF、CI、PCWP),以及cTnI、cTnI-mRNA、CaM和尿量的变化。结果治疗后,两组患者心功能指标均有所改善,且观察组明显优于对照组(P<0.05);治疗后,两组cTnI、cTnI-mRNA、CaM水平明显降低,尿量明显升高,且观察组上述指标明显优于对照组(P<0.05)。观察组治疗总有效率明显高于对照组,差异具有统计学意义(P<0.05)。结论生脉注射液能明显改善AMI后心源性休克患者心功能,降低血心肌蛋白水平,增加尿路,具有明显抗休克作用,可提高患者临床疗效。
Objective To explore the effect of Shengmai injection combined with routine treatment on cardiogenic shock after AMI and its effects on cardiac function,serum cTnI,cTnI-mRNA,CaM and urine volume.Methods A total of120cases of patients with cardiogenic shock after AMI treated in our hospital were collected and randomly divided into observation group and control group,with60cases in each group.The control group was given routine treatment,and the observation group was given Shengmai injection on the basis of the control group.The clinical effects,the cardiac function indexes(SBP,LVEF,CI,PCWP)and cTnI,cTnI-mRNA,CaM,urine volume were compared between the two groups.Results After treatment,the levels of cardiac function indexes of the two groups improved,and those of the observation group were significantly better than the control group(P<0.05).After treatment,the levels of cTnI,cTnI-mRNA and CaM increased,and urine volume decreased in the two groups,and those of the observation group were significantly better than the control group(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion Shengmai injection can significantly improve the cardiac function,reduce the levels of blood cardiac protein and increase urinary tract in patients with cardiogenic shock after AMI.And it has obvious antishock effect and can improve the clinical efficacy.
