摘要
目的探讨经皮冠状动脉介入(percutaneous coronary intervention,PCI)治疗对冠状动脉微循环阻力指数(microcirculation resistance index,IMR)及血清高敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)的影响。方法选取2015年2月至2016年3月在湖南湘潭中心医院治疗的稳定型心绞痛和不稳定型心绞痛患者85例,检测其PCI治疗前后IMR及hs-CRP浓度。结果患者PCI治疗后IMR(29.10±5.30 vs.20.14±4.28,P<0.05)和hs-CRP浓度[(4.40±0.73)mg/L vs.(1.24±0.23)mg/L,P<0.05]明显高于治疗前,差异有统计学意义。单支病变和多支病变患者PCI治疗后IMR和hs-CRP浓度均较治疗前增高,差异有统计学意义(P<0.05);多支病变患者PCI治疗前及治疗后IMR和hs-CRP浓度均高于单支病变患者,差异有统计学意义(P<0.05);稳定型心绞痛和不稳定型心绞痛患者PCI治疗后的IMR和hs-CRP浓度均较治疗前增高,差异有统计学意义(P<0.05);不稳定型心绞痛患者PCI治疗前及治疗后IMR和hs-CRP浓度均高于稳定型心绞痛患者,差异有统计学意义(P<0.05)。PCI治疗后IMR与hs-CRP浓度呈正相关(r=0.564,P<0.05)。结论稳定型心绞痛及不稳定型心绞痛患者IMR和hs-CRP浓度与冠状动脉病变程度有一定的联系,而PCI治疗可使冠状动脉粥样硬化性心脏病(冠心病)患者IMR和hs-CRP浓度升高。
Objectives To investigate the effect of percutaneous coronary intervention(PCI)on coronary artery micro?circulation resistance index(IMR)and serum concentration of high-sensitivity C-reactive protein(hs-CRP).Methods Totally85patients with stable angina pectoris(SAP)and unstable angina pectoris(UAP)were selected from February2015to March2016in Xiangtan Central Hospital.IMR and serum concentration of hs-CRP were de?tected before and after PCI.Results IMR(29.10±5.30vs.20.14±4.28,P<0.05)and serum concentration of hs-CRP[(4.40±0.73)mg/L vs.(1.24±0.23)mg/L,P<0.05]after PCI were significantly higher than those before treatment.Postoperative IMR and serum concentration of hs-CRP were higher than those preoperative in patients with single and multiple lesions(P<0.05);IMR and serum concentration of hs-CRP in patients with multiple lesions preoperative and postoperative were significantly higher than those in patients with single lesion(P<0.05).IMR and serum concentration of hs-CRP in SAP and UAP patients postoperative were higher than those before operation(P<0.05);Preoperative and postoperative IMR and serum concentration of hs-CRP in UAP patients were higher than in SAP patients(P<0.05).Postoperative IMR positively correlated with hs-CRP(r=0.564,P<0.05).Conclusions IMR and serum concentration of hs-CRP in SAP and UAP patients have a certain relationship with the degree of coronary artery disease,while PCI can increase IMR and serum concentration of hs-CRP in patients with coronary heart disease.
