氨氯地平阿托伐他汀钙复方片联合依折麦布对高血压患者血清中细胞因子的影响

Effect of amlodipine atorvastatin calcium compound tablets combined with ezetimibe on serum concentrations of cytokines in patients with hypertension

目的探讨氨氯地平阿托伐他汀钙复方片联合依折麦布治疗原发性高血压(高血压)的临床疗效及对血清中细胞因子人血清内皮素-1(endothelin-1,ET-1)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、C-反应蛋白(C-reactive protein,CRP)、白细胞介素-4(interleukin-4,IL-4)、白细胞介素-10(interleukin-10,IL-10)的影响。方法以入院病例号为编号,根据随机数字表法将110例明确诊断为高血压的患者随机分为对照组56例和观察组54例。观察组患者口服氨氯地平阿托伐他汀钙复方片,同时口服依折麦布;对照组患者口服氨氯地平阿托伐他汀钙复方片。比较两组用药后的疗效、安全性及血清中细胞因子TNF-α、CRP、IL-4和IL-10的浓度变化。结果治疗后,两组收缩压、舒张压较治疗前降低,差异有统计学意义(P<0.05);但组间收缩压、舒张压比较,差异无统计学意义(P>0.05)。观察组血清总胆固醇为(3.3±0.3)mmol/L、低密度脂蛋白胆固醇浓度为(1.3±0.3)mmol/L,均低于对照组,差异有统计学意义(P<0.05);观察组血清高密度脂蛋白胆固醇浓度为为(1.4±0.2)mmol/L,高于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清TNF-α、CRP、ET-1浓度较治疗前降低,观察组TNF-α、CRP、ET-1浓度低于对照组,差异有统计学意义(P<0.05);两组血清IL-10、IL-4浓度较治疗前升高,观察组IL-10、IL-4浓度高于对照组,差异有统计学意义(P<0.05)。结论氨氯地平阿托伐他汀钙复方片联合依折麦布可有效控制高血压患者血压水平和血脂浓度,有可能更好地发挥血管内皮功能保护作用。

Objectives To investigate the clinical efficacy of amlodipine atorvastatin calcium tablets combined with ezetimibe on serum concentrations of cytokines[endothelin-1(ET-1),tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-4(IL-4),interleukin-10(IL-10)]in patients with hypertension.Methods According to the random number table,110patients diagnosed as primary hypertension were randomly divided into control group(n=56)and observation group(n=54).Patients in observation group were given amlodipine and atorvastatin calcium tablets combined with ezetimibe;patients in control group were given amlodipine and atorvastatin calcium tablets.Clinical efficacy,safety and serum concentrations of ET-1,TNF-α,CRP,IL-4and IL-10after treatment were compared between the two groups.Results After treatment,systolic blood pressure(SBP)and diastolic blood pressure(DBP)of the two groups decreased(P<0.05),but there were no significant differences of SBP and DBP between the two groups(P>0.05).Serum concentration of TC was(3.3±0.3)mmol/L,and serum concentration of LDL-C was(1.3±0.3)mmol/L in observation group,which were lower than those of control group(P<0.05);Serum concentration of HDL-C was(1.4±0.2)mmol/L,which was higher than that of control group(P<0.05);After treatment,serum concentrations of ET-1,CRP,TNF-αwere significantly lower than those before treatment,and they were lower in observation group than in control group;serum concentrations of IL-10and IL-4increased than those before treatment,and they were higher in observation group than in control group(P<0.05).Conclusions Amlodipine atorvastatin calcium compound tablets combined with ezetimibe can effectively control blood pressure and blood lipid in patients with essential hypertension.It is possible to play a better role in the protection of vascular endothelial function.
