保留上睑动态解剖结构的切开重睑成形术

A Blepharoplasty to Keep the Dynamic Anatomy of the Upper Eyelid

目的:介绍一种创伤小,恢复快,术后能保留上睑动态解剖结构的切开法重睑术。方法:按常规方法设计重睑切口线和去除的皮肤量,在尽量保留真皮下血管的情况下去除眼轮匝肌,保留较多的睑板前疏松结缔组织和血管网,应用真皮腱膜固定法将切口下唇的真皮与睑板前筋膜组织固定缝合,重建提上睑肌肌束与上睑皮肤的连接,恢复自然的上睑动态解剖结构。切口精细解剖对位缝合。结果:2016年3月-2017年3月共52例患者使用此法施行重睑术,随访1个月~1年,所有患者均对手术效果满意,术后患者切口瘢痕轻,无凹陷,重睑形态自然,上睑动态效果接近自然。结论:恢复上睑动态解剖结构的切开重睑术创伤小、恢复期短、重睑动静态均自然,是值得推广的一种重睑术式。

Objective To introduce a small trauma and quick recovery method of blepharoplasty,which could reserve dynamic outcome of the upper eyelid.Methods The width of the double eyelid and the amount of skin removed were designed according to the normal principle.The orbicularis oculi muscle is removed as far as possible in the case of retaining capillary net,and more tissues of the tarsus and the vascular network were preserved.The dermis of the lower lip of the incision was sutured with pre-tarsus tissue,in order to reconstruct the anatomy between the levator palpebrae superioris and the upper eyelid skin,and restore the natural upper eyelid dynamic anatomic structure.The incision was finely dissected and sutured Results From March2016to March2017,a total of52patients were treated with this method to perform the double eyelid surgery.Followedup for1month to1year,all patients were satisfied with the post-operative effect.The dynamic double eyelid shape was close to natural eyelid without obvious scar.Conclusion This double eyelid surgery restores the dynamical construction of the upper eyelid,with less trauma,shorten recovery time,and natural shape including dynamic and static state,which is worth promoting.
