羊膜移植联合手术切除治疗初发翼状胬肉疗效分析

The Curative Effect Analysis of Amniotic Membrane Transplantation Combined with Excision in the Treatment of Primary Pterygium

目的:探究羊膜移植联合切除术在治疗初发翼状胬肉上的预后恢复情况以及美容效果。方法:将本院收治的143例(167眼)初发翼状胬肉患者作为研究对象,按照随机数表法分为观察组(73例,86眼)和对照组(70例,81眼)。对照组患者单纯采用胬肉切除术进行手术治疗;观察组患者采用胬肉切除配合羊膜移植治疗。对两组患者术后恢复情况及美观程度进行对比性分析。结果:治疗后1个月,观察组患者的患眼治愈率为96.51%,显著高于对照组的76.54%,差异有统计学意义(P<0.05)。观察组患者在治疗后1个月、3个月、6个月的患眼复发率分别为0.00%、2.33%、5.81%,均显著低于同期对照组(4.94%、9.88%、16.05%),差异有统计学意义(P<0.05)。观察组患者在术后出现瓣下积液的发生率分别为1.16%,未见有移植区充血以及移植瓣不透明发生,对照组中出现移植区充血、瓣下积液以及移植瓣不透明的机率分别为8.64%、6.17%、4.94%,差异有统计学意义(P<0.05)。结论:行羊膜移植配合切除术治疗初发翼状胬肉的患者预后恢复情况以及美容效果显著优于单纯行翼状胬肉切除术的患者,术后复发率更低,值得在临床推广应用。

Objective To research the prognosis and cosmetic effect of amniotic membrane transplantation combined with excision in the treatment of primary pterygium.Methods143patients(167eyes)with primary pterygium in our hospital were treated as the research objects.According to the random number table method,they were divided into the observation group(73cases,86eyes)and the control group(70cases,81eyes).The patients in the control group were treated with pterygium excision only,while the observation group were treated with pterygium excision combined with amniotic membrane transplantation.Compared and analyzed the postoperative recovery and aesthetic degree of the two groups.Results After1month of treatment,the cure rate of the eyes in the observation group was96.51%,significantly higher than that in the control group(76.54%),and the difference was statistically significant(P<0.05).The ocular recurrence rate in the observation group at1months,3months and6month after treatment were0.00%,2.33%,5.81%,were significantly lower than the control group(4.94%,9.88%,16.05%),the differences were statistically significant(P<0.05).The incidence of graft congestion in the observation group was1.16%,and there was no flap effusion and opaque flap transplantation,the incidence of graft congestion,hydrops under the flap and the opacity of the graft in the control group were8.64%,6.17%and4.94%,respectively,and the difference were statistically significant(P<0.05).Conclusion In the treatment of primary pterygium,the prognosis and cosmetic effect of patients undergoing amniotic membrane transplantation are significantly better than patients with simple pterygium excision,and the recurrence rate of pterygium is lower,which is worthy of clinical promotion and application.
