Q开关755nm皮秒激光联合强脉冲光治疗面部雀斑临床分析

Clinical Analysis of Q-switched 755nm Picosure Laser Combined with Intense Pulsed Light in the Treatment of Facial Freckles

目的:观察Q开关755nm皮秒激光联合强脉冲光治疗面部雀斑的疗效,探究综合治疗的新思路。方法:将150例门诊雀斑患者随机分成三组,每组50例。分别给予Q开关755nm皮秒激光(Q755nm)、强脉冲光(IPL)、Q开关755nm皮秒激光联合强脉冲光治疗,均治疗1次。治疗结束后第4周统计色素沉着发生率,第24周进行满意度评价。结果:治疗后4周及治疗后24周,Q755nm组疗效优于IPL组,联合治疗组疗效优于Q755nm组。患者对治疗的满意度评价,联合治疗组与单独治疗组无明显差异。Q755nm组有4例患者于治疗后2~4周皮损处出现炎症后色素沉着(PIH),联合治疗组有1例患者出现PIH,均于6个月内消退;IPL组无PIH病例出现。结论:Q开关755nm皮秒激光和IPL均能很好地治疗雀斑,Q755nm组疗效优于IPL组,而IPL治疗的安全性优于Q开关755nm皮秒激光,两者联合能够明显提高雀斑治疗的有效性和安全性。

Objective To explore a new method of treating freckles,by comparing the therapeutic effect of Q-switched755nm picosure laser,intense pulsed light(IPL)and the combined therapy in treating freckles.Methods A total of150cases of freckle patinets from outpatient were randomly assigned into3groups.Patients were given Q755nm,IPL and the combined therapy,respectively for one time.Curative effect was observed after4and24weeks.The rate of postinflammatory hyperpigmentation(PIH)was observed after4weeks.The satisfaction rate after24weeks was investigated.Results The improvement rate of Q755nm group was better than IPL group at4weeks and24weeks after treatment The improvement rate of combined group was better than Q755nm group.According to the satisfaction rate,there are no significant difference between combined group and Q755nm group or IPL group.There were4cases of patients out of50cases of group Q755nm developedPIH(8%),the combined group developed PIH(2%),which subsided in6months while no PIH occured after IPL therapy.Conclusion Both of Q-switched755nm picosure laser and IPL can treat facial freckles well.The efficacy of Q755nm picosure laseris better than that of IPL,while the safety of IPL is better than that of Q755nm.The combination therapy is superior in the clinical effect than each single method.
