白癜风患者血清免疫球蛋白及补体水平变化的研究

Clinical Significance on Level of Serum Immune Globulin and Complement in Patients with Vitiligo

目的:探讨不同分期不同分型白癜风患者血清免疫球蛋白及补体水平变化情况。方法:以2015年8月-2017年10月本院收治的226例白癜风患者为研究对象,按照疾病分期分为稳定期组(n=118)与进展期组(n=108),按照疾病分型分为节段型组(n=50)、局限型组(n=45)、散在型组(n=42)、泛发型组(n=49)及肢端型组(n=40),以同期的75例健康体检者为对照组。对白癜风患者进行药物治疗,采用免疫透射比浊法检测治疗前后白癜风患者及对照组的血清免疫球蛋白G(Ig G)、免疫球蛋白A(Ig A)、免疫球蛋白M(Ig M)、补体C3及C4水平。结果:稳定期患者的血清C3与C4水平均显著低于对照组(P<0.05),进展期患者的血清Ig G、Ig A、Ig M、C3及C4水平均显著低于稳定期组与对照组(P<0.05);节段型患者的血清Ig G、Ig A、Ig M、C3及C4水平均显著低于其他类型患者及对照组(P<0.05)。治疗后,稳定期患者的血清C3与C4水平均显著高于治疗前(P<0.05),进展期患者的血清Ig G、Ig A、Ig M、C3及C4水平均显著高于治疗前(P<0.05),节段型患者的血清Ig G、Ig A、Ig M、C3及C4水平均显著高于治疗前(P<0.05),各组患者血清Ig G、Ig A、Ig M、C3及C4水平比较差异无统计学意义(P>0.05)。结论:白癜风患者的血清免疫球蛋白及补体水平存在异常,不同分期不同类型白癜风患者的血清免疫球蛋白及补体水平也存在差异,通过治疗可调节其水平。

Objective To discuss the clinical significance on level of serum immune globulin and complement in patients with vitiligo.Methods226cases of vitiligo patients were taken as research objects in our hospital from August2015to October2017.All the patients were divided into stable phase group(n=118)and progressive stage group(n=108)according to staging of disease,all the patients were divided into segmental group(n=50),circumscribedg roup(n=45),scattered group(n=42),universal group(n=49)and acrak group(n=40)according to disease subtypes.75cases of healthy examined people were taken as control group in the same time.All the vitiligo patients were treated by drugs.The level of serum immune globulin G(IgG),immune globulin A(IgA),immune globulin M(IgM),complement C3and C4was detected by immunity transmission turbidity.Results The serum levels of C3,C4of stable phase group was significantly lower than that of control group(P<0.05),the serum levels of IgG,IgA,IgM,C3,C4of progressive stage group was significantly lower than that of stable phase group and control group(P<0.05).The serum levels of IgG,IgA,IgM,C3,C4of segmental group was significantly lower than that of other groups and control group(P<0.05).After treatment,the serum levels of C3,C4of stable phase group was significantly higher than before treatment(P<0.05),the serum levels of IgG,IgA,IgM,C3,C4of progressive stage group was significantly higher than before treatment(P<0.05),the serum levels of IgG,IgA,IgM,C3,C4of segmental group was significantly higher than before treatment(P<0.05).There was no statistical significance on the serum levels of IgG,IgA,IgM,C3,C4between groups(P>0.05).Conclusion The level of serum immune globulin and complement of vitiligo patients were abnormal.There was difference between different stages and types of vitiligo patients.Therapy could adjust the serum levels of serum immune globulin and complement.
