平台转换种植体在上颌美学区单牙种植修复中的应用效果

The Application Effect of Platform Switching Implants in the Single-Tooth Implant Restoration in Maxillary Aesthetic Area

目的:探讨平台转换种植体在上颌美学区单牙种植修复中的应用效果。方法:选择2012年3月-2014年11月笔者医院收治的86例(共100枚)上颌美学区单牙种植修复患者为研究对象,其中采用平台转换种植体连接的43例患者(48枚)为研究组,采用传统基台种植体连接的43例患者(52枚)为对照组,观察两组平均负荷时间、种植存留率、美学评价指标[红色美学指数(PES)、白色美学指数(WES)]及视觉模拟疼痛评分(VAS),随访两组修复后3年种植体边缘骨吸收量及牙周状况。结果:修复前两组PES、WES评分、VAS评分比较无显著性差异(P>0.05),修复后两组PES、WES评分较修复前明显升高,VAS评分显著降低,且研究组的改善幅度较对照组明显,有显著性差异(P<0.05)。研究组平均负荷时间较对照组显著缩短,种植存留率(100.0%)较对照组(88.4%)显著提高,3年后随访种植体边缘骨吸收量,研究组较对照组明显低,牙周不良状况发生率显著低于对照组,有显著性差异(均P<0.05)。结论:平台转换种植体在上颌美学区单牙种植修复中应用时具有负荷时间短、疼痛轻及美学和预后效果佳的显著优势。

Objective To explore the application effect of platform switching implants in the single-tooth implant restoration in maxillary aesthetic area.Methods From March2012to November2014,86cases(100teeth)receiving single-tooth implant restoration in maxillary aesthetic area in our hospital were studied.43patients(48teeth)treated by connection with platform switching implants were included into the study group and43patients(52teeth)with traditional abutment implants were included into the control group.The average loading time,planting survival rate,aesthetic evaluation indexes[pink esthetic scores(PES),white esthetic scores(WES)]and visual analogue scores(VAS)were observed.The marginal bone loss and periodontal condition were followed up in3years after restoration.Results Before the restoration,PES,WES and VAS showed no significant differences between the two groups(P>0.05).After the restoration,PES and WES were increased significantly and VAS scores were decreased significantly.The improvement in the study group was more significant than that in the control group(P<0.05).The average loading time was significantly shorter and the planting survival rate was significantly higher in the study group than the control group.The marginal bone loss was significantly lower in the study group than the control group at3years after the restoration.The incidence of poor periodontal condition was significantly lower in the study group than the control group(P<0.05).Conclusion The platform switching implants have advantages of short loading time,light pain,good aesthetic and prognosis in the single-tooth implant restoration in maxillary aesthetic area.
