个案护理对提高ART中MSM人群艾滋病相关知识和行为的效果

Study on the changes of knowledge and behavior among MSM population who are on ART treatments by case management

目的探讨个案护理对正在接受艾滋病抗病毒药物治疗(ART)的男-男同性(MSM)人群感染艾滋病患者相关知识和行为的干预效果。方法选取2014年7月至2015年7月在该中心就诊并接受ART的MSM人群感染艾滋病的90例患者,将其分为研究组和对照组,各45例。研究组进行个案护理,对照组进行常规模式护理。1年后,分别对2组人员艾滋病相关知识知晓率和安全套使用情况进行问卷调查,收集数据进行分析。结果 1年后,研究组40例完成随访,对照组41例完成随访。研究组"每次性行为时正确使用安全套能不能降低HIV传播的危险""与HIV感染者或患者一起吃饭会不会感染HIV""蚊虫叮咬会不会传播HIV"的知晓率高于对照组,差异均有统计学意义(P<0.05)。研究组最近6个月与异性发生性行为时使用安全套的构成比高于对照组,差异有统计学意义(P<0.05)。研究组同伴参检率高于对照组,差异有统计学意义(P<0.05)。结论个案护理可以促进ART治疗感染艾滋病的MSM人群艾滋病知识和行为改变,同时提高其同伴参检率。

Objective To study the intervention effectiveness of case management on the knowledge and behavior of men who have sex with men(MSM)who are on antiretroviral therapy(ART).Methods From July2014to July2015,a randomized controlled trial(RCT)was conducted among the MSM population who were receiving ART in Chongqing Infectious Disease Medical Center.The RCT was divided to the study group and the control group.Case management was conducted for the study group,the control group was given routine care.Follow?up survey was conducted for both groups to collect data for further analysis.Results A total of40people were included in the study group and41people were in the control group.The study group showed that improved knowledge level compared to the control group in terms of“whether accurate usage of condom can lower the risk of HIV transmission”“whether eating with people living with HIV can transmit HIV”and“whether mosquito bite can transmit HIV”,the differences were statistically significant(P<0.05).The study group also showed that higher frequency of condom use when having sex with the opposite sex in the last six months than that the control group,the difference was statistically significant(P<0.05).The peer particaipation rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Case management can improve ART among MSM and improve their knowledge of HIV and related behavior,including higher peer participation HIV testing rate.
