摘要
目的探讨男方年龄对IVF-ET妊娠结局的影响。方法回顾性分析2013年6月1日至2016年12月30日在我院行长方案IVF-ET助孕治疗的1 407例患者的临床资料,将患者按女方年龄分为<38岁和≥38岁,又根据男方年龄再分为<40岁和≥40岁,比较不同男性年龄组的患者一般情况(不孕年限、采卵数、移植日子宫内膜厚度、移植数)和临床妊娠结局(受精率、卵裂率、着床率、生化妊娠率、临床妊娠率)。结果当女方年龄<38岁时,男方年龄≥40岁组与<40岁组间一般情况和妊娠结局无明显统计学差异(P>0.05);女方年龄≥38岁时,男方年龄≥40岁组的着床率、生化妊娠率、临床妊娠率显著低于<40岁组(P<0.05);Logistics回归分析显示,校正女方年龄因素影响后,男方年龄≥40岁组与<40岁组临床妊娠率差异无统计学意义[OR=0.864,95%CI(0.597,1.251)];与夫妻双方年龄均<35岁组比较,男方年龄≥40岁且女方年龄≥38岁组临床妊娠率显著下降[OR=0.318,95%CI(0.178,0.568)](P<0.01)。结论在常规长方案IVF-ET周期中,控制女方年龄因素影响后,男方年龄对临床妊娠率无明显影响,但在男女双方均高龄状态下,男方年龄可能对临床妊娠率产生负面影响。
The data of1407couples undergone conventional IVF-ET with the standard long protocol for controlled ovarian hyperstimulation(COH)in our center from June1,2013to December30,2016were retrospectively analyzed.The patients were divided into two groups according to female age(<38years and≥38years),and each group subdivided into two groups based on male age(<40years and≥40years[DK(]).[DK)]The characteristics and pregnancy outcomes were compared among the different age groups.Results:There were no significant differences in the general condition and pregnancy outcomes between the male age≥40years and<40years group in the women’age<38years group(P>0.05).The implantation rate,biochemical pregnancy and clinical pregnancy in male’age≥40years group were significantly lower than those in male’age<40years groups when the women’age was≥38years(P<0.05).However,the difference of clinical pregnancy rate was not significant between the male’age≥40years and<40years group[OR=0.864,95%CI(0.597,1.251)]after adjustment for female age in logistic regression model.The clinical pregnancy rate in male’age≥40&fame age≥38group was significant lower than that in both male and female age<35years[OR=0.318,95%CI(0.178,0.568)](P<0.01).Conclusions:After adjustment for female age,male age had no significant influence on pregnancy outcomes in conventional IVF-ET.However,when both men and women are elderly,men’s age may still have a negative impact on clinical pregnancy rates.
