探讨集束化护理对鼻咽癌放疗后张口困难的预防作用

Objective to explore the preventive effect of cluster nursing on the difficulty of mouth opening after radiotherapy for nasopharyngeal carcinoma

目的:研究集束化护理对预防鼻咽癌患者放疗后张口困难的效果,为临床推广使用提供科学依据。方法:选取鼻咽癌放疗患者58例,随机均分为观察组和对照组,每组各29例。对照组给予常规化护理,观察组在对照组的基础上给予集束化护理。通过测量门齿距离观察评估张口困难程度,均于放疗前、放疗结束时、及放疗后1年内每3个月进行测量。结果:两组患者在接受放疗期间均发生不同程度的张口困难,但放疗期间及放疗结束时、放疗后3个月,两组患者的张口困难程度比较,差异均无统计学意义(P>0.05);观察组于放疗后6、9、12个月时,张口困难程度低于对照组,两组比较,差异有统计学意义(P<0.05)。结论:长期坚持采用集束化护理干预,可明显减少鼻咽癌放疗患者张口困难发生率,并减轻张口困难的严重程度。

Objective:To study the effect of cluster nursing on preventing the difficulty of mouth opening after radiotherapy in patients with nasopharyngeal carcinoma(NPC),so as to provide scientific basis for clinical application.Methods:A total of58patients with nasopharyngeal carcinoma treated in our hospital were randomly divided into the following two groups.The observation group of29cases were given routine nursing and cluster nursing,while the control group(n=29)received routine nursing intervention.By measuring the distance to evaluate the incisor trismus degree.Measurements were taken at the end of radiotherapy,at the end of radiotherapy,and every3months after radiotherapy.Results:Two groups of patients with different degrees of difficulty in mouth opening during radiotherapy.At the end of radiotherapy and at the end of radiotherapy,3months after radiotherapy,there was no significant difference in the degree of difficulty in mouth opening between the2groups(P>0.05).At6,9,12months after radiotherapy,the difficulty of mouth opening in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:Long term adherence to cluster nursing intervention can significantly reduce the incidence of difficulty in opening mouth in patients with nasopharyngeal carcinoma radiotherapy,and reduce the severity of mouth opening.
